膈下动脉供血在中晚期肝癌介入治疗中的价值

目的研究中晚期肝癌膈下动脉开放的DSA表现及高度选择性介入栓塞治疗的临床价值,以提高介入治疗的临床疗效。方法膈下动脉参与肝脏肿瘤供血18例,男15例,女3例,年龄34～67岁,全部为原发性肝癌。行膈下动脉造影确认供血范围后将导管超选择至供血支,对向肿瘤供血的膈下动脉分支行栓塞化疗。结果肝脏肿瘤均位于肝脏上部,接近膈肌或/和肝包膜。膈下动脉造影均为膈下动脉以终末支方式参与肿瘤供血,栓塞治疗成功。结论膈下动脉是中晚期肝癌的重要供血侧支动脉,认识其供血特点并正确处理在中晚期肝癌介入治疗中具有重要的意义。

The value of blood supply by inferior phrenic artery to advanced hepatic carcinoma in interventional embolization therapy

Objective To study the result of DSA for 18 cases with advanced hepatic carcinoma that there is bloody supply from inferior phrenic artery and the clinical value of super selective interventional embolization, so that to improve the clinical curative effects of interventional therapy. Methods Eighteen patients(15 males and 3 females, aged 34-67) with hepatic carcinoma, having inferior phrenic artery as the tumor-feeding artery. Were collected. The lesions of all paitents was primary hepatic cancinoma. Inferior phrenic arteriography was carried out when the fact that the tumor received blood supply from the inferior phrenic artery was confirmed during the interventional procedure. When the feeding range was clarified, selected catheterization of the feeding branch was done and TACE was followed. Results All lesions located in the posterior and subcapsular of the liver. Inferior phrenic artery catheterization revealed that these lesions drew blood supply from inferior phrenic artery in the 18 patients. All the blood supply of tumor were successfully embolized. Conclusions The inferior phrenic artery is usually one of the main collaterals in advanced hepatic cancer. Knowledge of this artery and proper treatment of tumor-feeding arteries are very important for a successful therapy of advanced hepatic carcinoma
