肝动脉化疗栓塞联合经皮肝穿瘤内无水乙醇注射治疗不能切除肝癌的疗效观察

目的评价肝动脉化疗栓塞(transcatheter hepatic arterial chemoembolization,TACE)联合B超引导下经皮肝穿瘤内注射无水乙醇(percutaneous ethanol injection,PEI)治疗不能切除肝癌的疗效。方法67例不能手术切除的肝癌分为TACE组(TACE治疗,29例)和TACE PEI组(TACE联合PEI治疗,38例)。比较2组AFP下降率、治疗有效率及1、2年生存率。结果TACE PEI组和TACE组AFP明显下降(下降>50%)率分别为47.4%和20.7%(<0.05);治疗有效率分别为81.6%和55.2%(<0.05);1年生存率分别为73.7%和48.3%(<0.05),2年生存率分别为31.5%和13.8%(<0.05)。结论TACE联合PEI是一种治疗不能切除肝癌的有效方法。

Combination of Transcatheter Arterial Chemoembolization and Percutaneous Ethanol Injection for Treatment of Unresectable Hepatocellular Carcinoma

Objective To evaluate the effectiveness of a combination of transarterial chemoembolization (TACE) and percutaneous ethanol injection(PEI)in the treatment of unresectable hepatocellular carcinoma(HCC).Methods 67 patients with unresectable HCC were randomly divided into 2 groups.29 cases were treated by TACE alone,the others were treated by combination of TACE and PEI.All patients were followed up by exam-inations of AFP,liver function and CT.Results The rates of the decline of serum AFP level in TACE PEI group and in TACE group were 47.4%%and 20.7%,respectively(<0.05).The rates of effectiveness in the two groups were 81.6%and 55.2%,respectivel(y<0.05).The 1 and 2 year survival rates(73.7%and 31.5%)in TACE PEI group were significant higher than those(48.3%and 13.8%)in TACE group(<0.05).Conclusion Com-bination of TACE and PEI is an effective method for unresectable hepatocellular carcinoma
