腹腔镜和开腹阑尾切除术在治疗穿孔性阑尾炎中的手术效果比较

目的观察比较穿孔性阑尾炎采用腹腔镜和开腹阑尾切除术治疗的临床效果。方法将穿孔性阑尾炎患者60例随机分为观察组和对照组各30例。对照组行开腹阑尾切除术,观察组行腹腔镜手术。观察2组手术时间、肛门排气时间、下床活动时间、住院时间、术后止痛药应用率、并发症发生率。结果观察组手术时间长于对照组,肛门排气时间、下床活动时间、住院时间均短于对照组,差异均有统计学意义(P<0.05);观察组术后止痛药使用率及并发症发生率均低于对照组,差异均有统计学意义(P<0.05)。结论穿孔性阑尾炎采用腹腔镜阑尾切除术治疗,可显著降低并发症发生率,缩短住院时间,促进患者术后康复,提高患者生存质量。

Effect comparison of laparoscopic and open appendectomy in the treatment of perforating appendicitis

Objective To compare the clinical effect of laparoscopic and open appendectomy in the treatment of perforating appendicitis. Methods 60 cases patients with perforating appendicitis were randomly divided into observation group and control group, each of 30 cases. Control group opened appendectomy, observation group underwent laparoscopic surgery. Observed operative time, anal exhaust time, ambulation time, hospitalization time, the application rate of postoperative analge- sics, the incidence rate of complication of two groups. Results In observation group, operative time was longer than that of control group, anal exhaust time, ambulation time, hospitalization time were shorter than those of control group, there were sig- nificant difference（ P 〈 0.05 ） ;The application rate of postoperative analgesics and the incidence rate of complication were low- er than those of control group, there were significant difference （ P 〈 0.05 ）. Conclusion Laparoscopic appendectomy in the treatment of perforating appendicitis can significantly reduce the incidence rate of complication, shorten hospitalization time, and promote the rehabilitation of patient,improve patient quality of life.
