超声多普勒引导下痔动脉结扎术治疗混合痔临床观察

为探讨超声多普勒引导下痔动脉结扎术（DG—HAL）治疗痔病的临床疗效，对30例混合痔患者采用DG—HAL（DG—HAL组）治疗，与外切内扎组35例以及吻合器痔上黏膜环切术（PPH组）30例比较其治疗效果、不良反应和并发症。结果显示，DG—HAL治疗混合痔在出血方面与其他两组无显著性差异，在术后疼痛方面DG-HAL组优于PPH组和外切内扎组，在术后水肿方面DG-HAL组与PPH组无显著性差异，与外切内扎组有显著性差异（P〈0．05）。在术后住院时间及术后恢复正常活动时间方面DG—HAL组短于PPH组和外切内扎组。但在治疗痔脱出方面DG-HAL组不及其他两组。结果表明，超声多普勒引导下痔动脉结扎术创伤小、疼痛轻、恢复快、安全性高、并发症少，是治疗混合痔的有效方法。

Clinical Observation of Mixed Hemorrhoids Treated by Doppler Guided Hemorrhoidal Artery Ligation

In order to discuss the clinical efficacy of mixed hemorrhoids treated by Doppler guided hemorrhoidal artery ligation （DG-HAL）, 30 cases underwent DG-HAL, 35 cases underwent external dissection and internal ligation,30 cases PPH. All the three groups were compared for clinical efficacy,adverse reactions and complications. The results showed that DG-HAL group showed no significant difference in bleeding from the other two groups, but was superior to the other two groups in postoperative pain. DG-HAL group showed no significant difference in postoperative edema from PPH group but showed significant difference from external dissection and internal ligation group（ P 〈0.05）. DG-HAL group had a shorter hospital stay and recovery time than the other two groups, but was inferior to them in treating prolapsed hemorrhoids. It is concluded that DG-HAL is effective for mixed hemorrhoids,with minor invasion, less pain, rapid recovery, high safety, and fewer complications.
