低位直肠癌柱状腹会阴联合切除术后针刺干预治疗的临床研究

为探讨低位直肠癌柱状腹会阴联合切除术后针刺干预治疗的临床价值，将72例行柱状腹会阴联合切除术治疗的低位直肠癌患者随机分为治疗组和对照组，各36例。两组患者术后均常规补液，预防感染及对症治疗，并早期给予肠内外联合营养支持。在此基础上，治疗组术后给予针刺干预治疗，对照组术后不给予针刺干预治疗。对比两组患者术后肠鸣音出现时间、肛门通气时间、并发症发生情况及营养指标的变化。结果显示，两组均无围术期死亡病例，治疗组在术后肠鸣音出现时间、肛门通气时间及腹胀、创口感染发生率方面均优于对照组，P〈0．05或P〈0．01；治疗组患者术后第7天前白蛋白（PA）浓度、淋巴细胞总数（TLC）均高于对照组，P〈0．05。结果表明，低位直肠癌柱状腹会阴联合切除术后进行针刺干预治疗能促进患者肠蠕动功能的恢复，有利于肠内营养支持治疗，从而更好地改善患者营养状况，提高其免疫功能，而且可降低并发症的发生率。

Clinical Study on Acupuncture Intervention Following Cylindrical Abdominoperineal Resection for Low Rectal Carcinoma

This study was to evaluate the clinical value of postoperative intervention with acupuncture for low rectal carcinoma patients, colleted the 72 cases who had received cylindrical abdominoperineal resection （APR） and divided randomly into treatment group and control one,for each;both groups were given with routinely replenishing fluid, anti-infective treatment, symptomatic treatment, symptomatic treatment and early enternal and parenteral nutrition, on the basis, treatment group was also with acupuncture intervention after operation,while the control one was not given the intervention treatment;then the time for postoperative rugitus sound appearing and time of anal ventilation,the incidence of complication,and the nutri- tion indexes changes were compared between both groups.As results,there was no death cases during perioperative period in both groups and treatment group was all superior to control group in the time for postoperative rugitus-sound appearig and anal ventilation,abdominal distention and the incidence of wound infection（ P 〈0.05 or P 〈0.01） ;and,on the 7th after operation the level of prealbumin,the total lymphocyte count in treatment group were higher than in control group（ P 〈0.05）.Results show that acupuncture in- tervention following cylindrical APR can promote enterocinesia function recovery of low rectal carcinoma patients,so helping to intraintestinal alimentation support therapy, thus better improve patient＇s nutrition status and lift their immune function, as well as reduce the incidence of complication.
