虚实挂线配合对口切除旷置引流治疗高位复杂性肛瘘

捧要为探讨治疗高位复杂性肛瘘较为有效的手术方式．采用虚实结合挂线配合对口切除旷置引流治疗高位复杂性肛瘘30例（治疗组），并与传统的切开挂线术治疗的30例（对照组）进行比较。结果显示，两组患者在治愈率方面无显著差异；在愈合时间和术后肛门功能方面治疗组优于对照组（P〈0．05）。治疗组手术前后肛管压力改变以及术后肛门功能改变（肛门不完全失禁）的发生率明显低于对照组；治疗组平均治愈时间为26．5d，对照组为33．4d．两组比较有统计学意义（P〈0．05）。结果表明，虚实结合挂线配合对口切除旷置引流治疗高位复杂性肛瘘在缩短愈合时间、保护肛门功能及舡周皮肤完整性方面有较好效果。

Tight and Loose Thread-drawing plus Counter-excision and Exclusion Drainage for High Complex Anal Fistula

In order to explore an effective surgical procedure for high complex anal fistula, 30 cases received tight and loose thread-drawing plus counter-excision and exclusion drainage,and another 30 cases underwent conventional incision thread-drawing procedure. Compared the two groups. The results showed that no significant difference was found between two groups in curative rate; treatment group was superior to control group in wound healing time and postoperative anal function（ P 〈0.05）. The incidence of changes of pressure in anal canal and in anal function such as incomplete incontinence of anus,was significantly lower in treatment group than that in control group; the average healing time was 26.5d in treatment group and 33.4d in control group,showing statistically difference（ P 〈0. 05）. It is concluded that for high com- plex anal fistula tight and loose thread-drawing plus counter-excision and exclusion drainage can effectively shorten wound healing time,protect anal function and skin integrity around the anus.
