健脾中药对进展期胃癌生存期及转移复发状况的作用

目的 :观察健脾中药对进展期胃癌生存率和转移复发率的影响。方法 :对进展期胃癌 14 5例随机分成健脾中药组、化疗组、中西医结合组 ,比较 1、3、5年生存率和 1、2、3年累计转移复发率。并采用裸鼠脾包膜下人胃癌细胞穿刺接种实验性转移模型 ,观察健脾中药组、四君子汤组、对照组肝转移、腹腔转移、腹水出现率。结果 :临床研究显示 ,健脾中药组第 3年生存率高于化疗组 (P <0 0 5 ) ,第 5年生存率高于化疗组和中西医结合组 (P <0 0 5 )。健脾中药组第 2年转移复发率低于化疗组 (P <0 0 5 ) ,第 3年转移复发率低于化疗组和中西医结合组 (P <0 0 1、P <0 0 5 )。实验研究显示 ,健脾中药组、四君子汤组肝转移率和肝转移灶均数比较低于对照组 (P <0 0 5 ) ,腹膜种植转移灶均数比较健脾中药组低于对照组 (P <0 0 5 ) ,腹水率健脾中药组低于对照组 (P <0 0 5 )。结论 :健脾中药对进展期胃癌有延长生存期和抑制转移复发作用。

Effect on the Living Period, Metastasis and Recurrence of the Advanced Gastric Cancer by the Traditional Chinese Medicine of Jianpi

Objective :To observe the effect on the rate of survival and metastasis and recurrence of advanced gastric cancer by the traditional Chinese medicine of Jianpi. Methods :We randomly divided 145 cases of advanced gastric cancer patients into Jianpi group,Chemotherapy group and Integrated group and compared the survival rate of 1,3,5 years and the accumulative metastasis and recurrence rate of 1,2,3 years.We used the experimental metastasis model that the human gastric cancer cells were grafted onto the spleen envelope of the nude mice to observe the rate of liver metastasis,abdominal cavity metastasis and the ascites in the Jianpi group,Sijunzi Decoction group and control group. Results :The results of clinical study showed that the survival rate of 3 years of the Jianpi group was higher than that of Chemotherapy group( P <0 05) and the survival rate of 5 years of the Jianpi group was higher than that of Chemotherapy group and Integrated group ( P <0 05).The metastasis and recurrence rate of 2 years of the Jianpi group was lower than that of Chemotherapy group ( P <0 05) and the metastasis and recurrence rate of 3 years of the Jianpi group was lower than that of Chemotherapy group and Integrated group ( P <0 01, P <0 05).The results of experimental study showed that the rate of liver metastasis and the mean value of liver metastasis foci of Jianpi group and Sijunzi group were lower than those of the control group ( P <0 05).The mean value of metastasisc foci grafted onto the peritoneum and the rate of ascites of Jianpi group were lower than those of the control group( P <0 05). Conclusion :The traditional Chinese medicine of Jianpi has the effect on the elongation of living period and the inhibition of the metastasis and recurrence of the advanced gastric cancer patients.
