腹腔镜卵巢电灼打孔术配合药物促排卵治疗多囊卵巢综合征

目的 :探讨难治性多囊卵巢综合征 (PCOS)的治疗方法 ,以提高排卵率及恢复正常月经周期。方法 :选择药物治疗时间在 2年以上未妊娠的PCOS患者 ,经腹腔镜行多囊卵巢电灼打孔术 ,术后配合应用促排卵药物 ,观测血FSH、LH、T、P、E2 的变化及排卵率。结果 :排卵率达 89% ,受孕率为 6 8%。术后LH、T、E2 浓度均较术前明显下降 ,与术前平均值比较 ,均P <0 .0 1,术后受孕者 ,各种激素下降幅度较大 (P <0 0 1) ,病程较长者手术治疗效果较好。结论 :本疗法简单易行 ,创伤小 ,康复快 ,可重复多次进行 ,患者易接受 ,为困难的PCOS患者提供了新的治疗途径

The treatment of polycystic ovary syndrome in ovulation by using laparoscopic ovarian electrocauterization with drug

Objective:To study the treatment ways of the clomiphene resistant polycystic ovary syndrome (PCOS) so as to raise the ovulation rate and recover normal menses cycle. Methods:No pregnant patients with PCOS who had been given medication for more than two years were treated by laparoscopic ovarian electrocauterization and applied drugs of promoting ovulation after operation. They were observed the changes of blood FSH?LH?T?P?E 2 and the ovulation rate.Results:The ovulation rate was 89%,the pregnancy rate was 68%. There was more significant reduction in the level of blood LH?T?E 2 after operation than before operation (P<0.01). The pregnant women′s various hormonal level was declined greatly (P<0.01); the effect was well especially for the patients with long course of diseases. Conclusions:The laparoscopic ovarian drilling is easily accepted by patients because it has several advantages of easier conducted ,small wound, quickier recovery and repeatable . It offers a new treatment way for hard cured patients with PCOS.
