摘    要:目的:探讨改良肾镜联合电切镜气压弹道碎石术治疗膀胱结石的方法及疗效。方法:采用改良肾镜联合电切镜气压弹道碎石术治疗膀胱结石27例,其中男性26例,女性1例,年龄18~83岁,平均年龄(66.8±14.7)岁。结石单发15例(55.6%),多发(最多5枚结石)12例(44.4%),结石长径0.8~5.0 cm,平均(2.1±0.7)cm。结果:所有患者均一次性碎石成功,无1例结石残留,结石清除率100%,碎石时间20~165 min,平均(45.9±33.9)min。术中无严重并发症发生,1例术后出现尿道狭窄,经尿道扩张后治愈。结论:改良肾镜联合电切镜气压弹道碎石术治疗膀胱结石具有高效、安全、创伤小、效果确切等优点,适合临床推广应用。

Abstract:Objective:To observe the curative effect of modified pneumatic lithotripsy via nephroscope and resectoscope for treatment of bladder calculus.Methods:Twenty-seven patients with bladder calculus underwent modified pneumatic lithotripsy via nephroscope and resectoscope,in whom 26 were male and 1,female.The age ranged from 18 to 83 with an average of(66.8±14.7) years.Fifteen were single stone(55.6%) and 12 multiple calculi(44.4%).The stone measured minimally 0.8 cm and maximally 5.0 cm with 2.1±0.7 in major diameter.Results:The stones were totally crushed at one-time and no single residual calculus was found in any patient,with clearance rate of 100%.The procedure lasted from 20 to 165 min with a mean time of(45.9±33.9)min.No serious complication occurred intraoperatively.Although postoperative urethral stricture occurred in one case,yet it was managed with urethral dilatation.Conclusion:Modified pneumatic lithotripsy assisted with nephroscope and resectoscope can be effective for treatment of bladder calculus and recommended for clinical application for its safety and minimal invasion.
