病毒性脑炎高压氧治疗前后血清NSE的变化及其意义

摘    要:目的探讨神经元特异性烯醇化酶(neuron—specific—enolase,NSE)在病毒性脑炎患儿血清中的变化,旨在评价高压氧治疗病毒性脑炎的临床价值。方法66例病毒性脑炎患儿被随机平均分为两组:高压氧组和常规治疗组,两组均于治疗前、治疗后14d进行NSE测定,另有10名健康儿作为正常儿组。也进行NSE测定。NSE用放射免疫法进行测定。结果治疗前NSE水平:高压氧组:(19.20±4.62)μg/L;常规治疗组:(19.25±4.23)μg/L;正常儿组:(7.45±2.18)μg/L。治疗后NSE水平:高压氧组:(7.51±2.02)μg/L;常规治疗组:(9.32±2.05)μg/L;正常儿组:(7.28±2.04)μg/L。病毒性脑炎患儿治疗前NSE水平明显增高,与正常儿组比较差异有统计学意义(P〈0.05);治疗14d后病毒性脑炎患儿NSE水平明显下降,与治疗前比较差异有统计学意义(P〈0.05),且高压氧组下降得更为显著;高压氧组与常规治疗组比较.治疗前两者NSE水平差异无统计学意义(P〉0.05),治疗后与常规治疗组比较差异具有统计学意义(P〈0.05)。结论常规治疗配合高压氧治疗可更有效改善病毒性脑炎患儿血NSE水平,促使受损脑细胞尽快恢复。

Abstract:Objective To monitor the changes in serum neuron-specific enolase (NSE) level in children with viral encephalitis before and after hyperbaric oxygen (HBO) treatment, so as to assess the applicable value of HBO in the treatment of children with viral encephalitis. Methods Sixty-six children with viral encephalitis were evaluated. They were randomly divided into two groups, i.e. HBO group treated with HBO and group treated with routine treatment ( without HBO ). The serum NSE levels of the two groups were measured hefore treatment and 14 days after treatment. In addition, 10 health children were selected as control group. Teir serum NSE levels were measured with radioimmunolgical method. Results The NSE levels of the children before treatment were ( 19.20 ± 4.62 )μg/L ( HBO group) , ( 19.25 ± 4.23 )μg/L ( routine group) and ( 7.45± 2.18 ) μg/L( control group). The NSE levels of those after 14 days' treatment were ( 7.51± 2.02 )μg/L ( HBO group) , ( 9.32 ±2.05 ) μg/L ( routine group) and ( 7.28 ± 2.04 ) μg/L ( control group ). The NSE levels of the children with viral encephalitis increased significantly before treatment when compared with that of the control group ( P 〈 0.05 ). After 14 days' treatment, the NSE levels of the children with viral encephalitis decreased significantly ( P 〈 0.05 ) , and the decrease in the NSE levels of HBO the group was more significant. There was a significant difference after ten days' treatment between the HBO group and the routine group ( P 〈 0.05 ). Conclusion Routine treatment combined with hyperbaric oxygen treatment can more effectively ameliorate the NSE levels of children with viral encephalitis, and accelerate the recovery of damaged cerebral cells as soon as possible.
