吸烟者内皮型一氧化氮合酶基因G894T多态性与冠心病的相关性研究

摘    要:目的探讨吸烟者内皮型一氧化氮合酶基因(eNOS)G894T多态性与冠状动脉粥样硬化性心脏病(冠心病)的发生及其严重程度间的关系。方法将203例吸烟者和182例非吸烟者根据冠状动脉造影结果分为冠心病组(196例)和对照组(189例),以冠状动脉病变支数判定严重程度。以聚合酶链式反应-限制性片段长度多态性(PCR—RFIP)检测eNOS基因G894T多态性,按吸烟与否分析G894T多态性与冠心病的关系。结果冠心病组GT+TT基因型分布与T基因突变频数明显高于对照组(x^2=4.26、6.21,P均〈0.05);吸烟者基因GT+TT型及T等位基因频率均显著高于非吸烟者(x^2〈5.82、5.77,P均〈0.05);冠心病组eNOS基因型分布在单支与多支病变组之间差异无统计学意义(x^2=3.36,P〉0.05),而吸烟的冠心病患者差异有统计学意义(x^2=6.48,P〈0.05),吸烟的GG+TT型者更可能发生多支病变,OR=3.42,95%CI(1.440—4.400)。结论eNOS894位点G→T、吸烟与冠心病的发生及其严重程度间的关系密切。

Relative study among the G894T polymorphism of endothelial nitric oxide synthase gene, smoking and coronary artery disease

Abstract:Objective To investigate the relationship between the eNOS G894T polymorphism and the presence and the severity of coronary artery disease (CAD) concerning with smoking. Methods All patients were divided into two groups of CAD and non CAD, including 203 smokers and 182 cases without smoking by coronary angiograms. The severity of CAD was expressed by the number of affected vessels. Polymerase chain reaction (PCR) with restriction fragment length polymorphism analysis was used to detect the G894T variant of the eNOS gene in 196 patients with CAD and 189 controls. The relationship between CAD and G894T polymorphism was analyzed based on smoking or not. Results The frequency of the eNOS GT and TT genotypes in the CAD group was more than that in control group (x^2=4.26,P 〈 0.05 ), allele frequency was significantly different (x^2=6.21,P 〈 0.05);there were significantly differences between smokers and those without smoking too(x^2=5.82,5.77,P 〈 0.05); the frequency of the eNOS GT and TT genotypes between the different number of affected vessels was not significantly different in CAD (x^2=3.36, P 〉 0.05 ), but it was significantly different in smokers with CAD (x^2=6.48, P〈 0.05); the coexistence of the 894 GT or TT genotypes and smoking led to an increased risk of severe CAD, OR=3.42,95%CI 1.440-4.400. Conclusions These findings suggest that the G894T polymorphism of the eNOS gene, smoking were associated with the presence and the severity of CAD.
