微粒栓塞剂化疗栓塞治疗肝转移癌的疗效观察

目的 评价微粒栓塞剂化疗栓塞肝转移癌患者的临床疗效。方法 回顾性分析36例自2009年6月至2011年6月在我院应用350-560μm明胶海绵微粒化疗栓塞肝转移癌患者的临床资料，通过评价肿瘤反应、肝功能损伤、不良反应及生存率评价临床疗效。结果 中位随访时间15个月，栓塞病灶均有不同程度坏死及缩小，36例患者完全缓解(CR)：2例；部分缓解（PR）：27例；稳定（SD）：5例；进展（PD）2例；客观反应率（ORR）为80.55%；术后7天肝功能基本恢复至术前水平（p＞0.05），无严重并发症的发生；6个月生存率91.6%、12个月生存率83.3%。结论 明胶海绵微粒化疗栓塞肝转移癌取得良好的近期疗效，远期疗效还有待于进一步观察。

Efficacy of gelatin sponge particles chemoembolization in the treatment of metastatic liver cancer

0bjective Evaluate the clinical efficacy of transarterial chemoembolization with gelatin sponge particles(GSPs-TACE) in metastatic liver cancer patients. Methods From June 2009 to June 2011, 36 patients with metastatic liver cancer were enrolled to undergo GSPs -TACE. Tumor responses were evaluated by computed tomography according to the modified Response Evaluation Criteria in Solid Tumors (mRECIST). Survival rates was performed using Kaplan-Meier estimator testing. Results After a median follow-up of 16 months, the lesions varied degree of necrosis and narrow. According to mRECIST, complete response (CR), partial response (PR), stable disease (SD) and progressive disease (PD) were, 2, 27,5 and 2 cases, respectively. Obective response rate (ORR) was 80.6%. There were no significant differences in liver function between pre- and 7d post-GSPs-TACE (P> 0.05). Severe complications post-TACE, liver failure, liver abscess and biloma, were not seen in this series. The 6, and 12-month survival rates were 91.5%, and 43.3%, respectively. Conclusion Gelatin sponge particles chemoembolization metastatic liver cancer achieved good short-term effect, the long-term efficacy remains to be seen.
