摘    要:鉴别诊断胶质瘤复发与治疗后反应一直是临床关注的热点和难点。胶质瘤治疗后反应主要包括假性进展、放射性坏死及假性反应。常规影像学方法主要用于观察肿瘤形态学特点,不能评价残存肿瘤活性,鉴别胶质瘤复发与治疗后反应的价值有限。包括弥散成像、灌注成像、磁共振波谱、化学交换转移饱和成像及PET等在内的影像学新技术有助于理解胶质瘤复发与治疗后反应细胞结构、血流动力学、代谢及生物学功能的差异,提高评估胶质瘤治疗后反应的准确性。本文就影像学新技术评估胶质瘤治疗后反应的研究进展进行综述。

Abstract:Differentiating recurrence and post-treatment responses of glioma has always been a clinic hot and difficult topic. Post-treatment effects of glioma mainly include pseudoprogression, radiation necrosis and pseudoresponse. Conventional imaging methods were mainly used to observe the morphological characteristics of gliomas, but not to evaluate the activity of residual tumors for their limited value of differentiating glioma recurrence from treatment effects. Imaging novel techniques, including diffusion weighted imaging, perfusion weighted imaging, MR spectroscopy, chemical exchange transfer saturation imaging and PET, could help to understand the differences of cellular structures, hemodynamics, metabolisms and biological functions between glioma recurrence and treatment effects, therefore improve the accuracy of assessing post-treatment response in gliomas. The research progresses of imaging novel techniques in evaluation on post-treatment effects of gliomas were reviewed in this article.
