床旁超声心动图结合高频超声诊断新生儿肺动脉瓣闭锁

摘    要:目的 观察床旁超声心动图结合高频超声诊断新生儿肺动脉瓣闭锁(PA)的价值。方法 回顾性分析34例床旁超声心动图疑诊为PA的新生儿,结合高频超声综合评估肺动脉及右心室发育情况及其他先天性心脏发育畸形。结果 34例疑诊PA新生儿中,结合高频超声诊断30例PA,其中1例漏斗部肥厚闭锁伴肺动脉瓣及主干发育不良,9例肺动脉瓣膜性闭锁,19例肺动脉瓣未发育,1例肺动脉瓣及主干缺如伴分支肺动脉异常起源;4例诊断为功能性PA;均合并心房水平分流及动脉导管未闭。结论 床旁超声心动图检查结合高频超声对鉴别PA及其分型具有重要意义。

Abstract:Objective To observe the value of bedside echocardiography combined with high-frequency ultrasound for diagnosis of neonatal pulmonary atresia (PA). Methods Data of 34 neonates with suspected PA detected with bedside echocardiography combining high-frequency ultrasound were retrospectively analyzed, and the development of pulmonary artery, right ventricle and other congenital heart development abnormalities were observed. Results Among 34 neonates, 30 were diagnosed PA with bedside echocardiography combining high-frequency ultrasound, including 1 hypertrophic atresia of the funnel with dysplasia of the pulmonary valve and trunk, 9 pulmonary valve-like atresia, 19 pulmonary valve non-development and 1 absence pulmonary artery trunk with abnormal origin of pulmonary artery branch, whereas functional PA was diagnosed in 4 cases. All 30 cases complicated with horizontal shunt of atrium and patent ductus arteriosus. Conclusion Bedside echocardiography combined with high-frequency ultrasound was of great value in distinguishing and classification of PA.
