视网膜色素变性不同证型红细胞免疫功能及T淋巴细胞表面抗原表达的改变

摘    要:目的]研究原发性视网膜色素变性(RP)的自身免疫发病机理及与中医证型的关系.方法]对52例不同中医证型的原发性视网膜色素变性患者及22例健康人进行红细胞免疫功能的两项指标即红细胞c3b受体花环结合率(RCR)、免疫复合物花环结合率(RICR),及免疫复合物(CIC)、外周血T淋巴细胞表面人类白细胞DR抗原(HLA-DR)表达率的测定.结果]RP患者RCR、RICR均降低,CIC值增高,HLA-DR表达增高,与正常组比较差异有显著性(P<0.05或P<0.01);RP患者的RCR与CIC呈显著负相关关系(P<0.01);而脾肾阳虚组与肝肾阴虚组比较,RCR、RICR下降更低、CIC值和HLA-DR表达更高,差异均有显著性(P<0.01).结论]RP患者红细胞免疫功能原发性低下,CIC和HLA-DR抗原表达增高,提示了自身免疫性疾病RP因虚致瘀的发病过程.

Abstract:To investigate the relationship between autoimmune pathogenic mechanism of retinal pigmentation (RP) and syndrome patterns in traditional Chinese medicine.Erythrocytic C3b receptor rosette(RCR) and erythrocytic immunocomplex rosette(RICR), circulating immune complex (CIC) content and expression rate of human leukocyte antigen D receptor (HLA-DR) in T lymphocyte of peripheral blood were detected in 22 healthy volunteers (group A) and 52 cases of RP (group B).In group B,RCR and RICR were decreased and CIC content and expression rate of HLA-DR were increased compared with group A (P<0.05 or P<0.01);RCR had negative correlation with CIC content (P<0.01). The decrease of RCR and RICR and the increase of CIC content and expression rate of HLA-DR in RP cases with yin-deficiency of liver-kidney (pattern 1) differed much from those with yang-deficiency of spleen-kidney (pattern 2) (P<0.01).Conclusion]Erythrocytic immune function decreases in RP;the increase of CIC content and expression rate of HLA-DR indicates the autoimmune pathogenic mechanism of RP with deficiency blood-stasis.
