微探头超声对食管隆起性病变的诊断价值

摘    要:目的 探讨微探头超声对食管隆起性病变的诊断价值.方法 在内镜引导下应用10 MHz微型超声探头对82例食管隆起性病变行微探头超声检查.结果 82例食管隆起性病变中微探头超声检出息肉6例,食管炎4例,平滑肌瘤19例,平滑肌肉瘤2例,静脉瘤15例,脂肪瘤3例,囊肿6例,食管癌10例,外压性病变17例 (其中15例主动脉压迹,1例肺肿瘤压迹,1例脊椎压迹),27例经病理证实.结论 微探头超声能够显示食管层次结构,对食管隆起性病变能够进行组织层次定位,较好地提示病变的性质,对食管隆起性病变的诊断与鉴别诊断有较高的价值.而且能够通过狭窄的食管,扩大了临床应用范围.

Abstract:Objective To investigate the diagnostic value of miniprobe ultrasonography (MPS) in patients with esophageal protruding lesions. Methods Eighty-two patients with protruding lesions of esophagus were examined by MPS. Results The diseases and conditions in 82 patients included: 6 esophageal polyps, 4 inflammatory protrudings, 19 esophageal leiomyomas, 2 esophageal leiomyosarcomas, 15 esophageal venous aneurysms, 3 esophageal lipomas, 6 esophageal cysts, 10 esophageal carcinomas and 17 extra esophageal compressions (15 aorta compressions, 1 lung tumor compression, 1 spine compression). Twenty-seven patients were verified by surgeries and pathologies. Twenty-five patients were followed-up by MPS. Conclusion MPS can image the esophageal layer, locate the histological layer of esophageal eminent lesions and observe their ultrasonographic characteristics. It has certain value in the diagnosis and differential diagnosis of protruding eminent lesions of esophagus. Furthermore, being miniature probe, MPS can pass the strictured esophagus, which is widely applicable.
