4种肿瘤标记物对大肠癌定性诊断价值的研究

探讨唾液酸(SA)、多胺(PA)、癌胚抗原(GEA)和糖蛋白抗原242(CA242)等肿瘤标记物(TM)单项和多项联合检测在大肠癌定性诊断中的价值。方法应用HP％M高效液相色谱仪和HPl040A荧光检测器或酶联免疫吸附法测定70例大肠癌和68例健康成人血清中的SA、PA、CEA、CA242水平，分别计算单项和多项TM联合检测对大肠癌的敏感性、特异性、阳性预测率、阴性预测率和总有效率。结果4种TM在大肠癌病人中均较健康成人显著升高，单项指标以CA242的诊断价值最高，其次是PA。4种TM联合检测，其敏感性明显升高，SA+PA+CA242组合敏感性可达100％，但特异性为63.46％。 结论4种TM均是诊断大肠癌的有价值的标记物，多项指标联合检测提高了检测敏感性，可作为人群筛查大肠癌的方法。

VALUE OF 4 TUMOR MARKERS IN DIAGNOSIS OF COLORECTAL CANCER

To evaluate the value of single detection and combined detection of sialic acid (SA), polyamine(PA), carcinoemhryonic antigen (CEA) and carbohydrate antigen 242(CA242) in the qualitative diagnosis of colorectal cancer. Methods Serum levels of SA,PA,CEA,CA242 of 70 cases of colorectal cancer and 68 healthy adults were detected with HP%M high performance liquid chromatography, HP1040A fluorescence detector or enzyme-linked immunoabsorbent assay. Sensibility. specificity, positive predictive rate,negative predictive rate, and total efficiency of single detection or combined detection of 4 TMs were calculated respectively. Results Circulating TMs raised significantly in patients with colorectal cancer, CA242 detection had the highest availability, combined detection of SA+PA+CA242 increased sensibility of 100%, but the specificity was only 63.46%. Conclusion Four TMs are all valuable in diagnosis of colorectal cancer. Combined detection increases diagnosis sensibility and is a good choice in population screen of colorectal cancer.
