伴有高血压的老年牙周炎患者牙周基础治疗和维护治疗的疗效观察

目的：探讨并评价定期牙周基础治疗对伴有高血压的老年牙周炎患者的临床疗效。方法：选取21例高血压且口服钙拮抗剂类降压药的老年牙周炎患者,对其进行牙周基础治疗,并于初次治疗后6、12个月分别再进行一次性洁治和刮治,检查并记录患者治疗前（基线）、基础治疗后1、6个月和12个月的各临床指数,包括菌斑指数（plaque index,PLI）、龈沟出血指数（sulcus bleeding index,SBI）、牙周探诊深度（probing depth,PD）和牙龈增生指数（gingival thickness index,GT）。结果：牙周基础治疗后1、6个月和12个月,21例患者的牙周状况都明显改善,PLI、SBI和PD与治疗前相比,差异有统计学意义（P〈0.05）,GT为2级以上的位点恢复至1级以下的百分比分别为：治疗后1个月为52.3%,6个月为76.1%,12个月为92.7%。结论：对于伴高血压的老年牙周炎患者,牙周基础治疗及每6个月的维护治疗可以很好地控制牙周组织炎症,并能使与口服降压药有关的牙龈增生得到恢复。

Clinical observation of initial and maintenance periodontal therapy on elderly periodontitis patient with hypertension

Objective：To investigate and evaluate the treatment effectiveness of the regular initial periodontal therapy on elderly periodontitis patient with hypertension.Methods：Selected 21 patients with high blood pressure and oral calcium antagonists older antihypertensive drugs in patients with periodontitis, periodontal treatment of their conduct, and in the first months after treatment were 6.12 one-time rescaling and scaling, check and record the patients before treatment （baseline）, after basic treatment, 1,6 months and 12 months of clinical indices, including plaque index （plaque index, PLI）, gingival sulcus bleeding index （sulcus bleeding index, SBI）, tooth weeks of probing depth （probing depth, PD） and gingival hyperplasia index （gingival thickness index, GT）.Results：Periodontal treatment 1,6 months and 12 months, 21 patients were improved periodontal status, PLI, SBI and PD compared with that before treatment was significantly （P0.05）, GT 2 and above the site restored to 1 percent below, respectively：1 month after treatment was 52.3%, 76.1% at 6 months, 12 months was 92.7%.Conclusion：For patients with hypertension in elderly patients with periodontitis, periodontal treatment and every 6 months maintenance therapy can control the periodontal tissue inflammation, and make oral antihypertensive agents with the restoration of gingival hyperplasia.
