高敏C反应蛋白及血压变异性与经皮冠状动脉介入术后心肌重构的关系

目的探讨高敏C反应蛋白(hypersensitivity C-reactive protein,hs-CRP)及血压变异性与经皮冠状动脉介入术后心肌重构的关系。方法行经皮冠状动脉介入术治疗的慢性稳定性心绞痛患者43例,依据术前hs-CRP水平分为低hs-CRP组(hs-CRP≤3mg/L)20例,高hs-CRP组(hs-CRP>3mg/L)23例。分别于术前及术后12个月行超声心动图检查,测定左心室舒张末期内径、左心室舒张末期容积、左心室收缩末期容积及左心室射血分数,并进行比较。结果 hs-CRP与血压变异性呈正相关;2组治疗后左心室舒张末期容积、左心室收缩末期容积、左心室射血分数均较治疗前增高(P<0.05);高hs-CRP组左心室舒张末期内径较治疗前增大明显(P<0.05)。结论 hs-CRP与血压变异性呈正相关,与患者术后心肌重构有相关性。

Relationship of high sensitivity C-reactive protein and blood pressure variability with ventricular remodeling after coronary stent implantation

Objective To evaluate the relationship of high sensitivity C-reactive protein(hs-CRP) and blood pressure variability(BPV) with ventricular remodeling after coronary stent implantation and its significance.Methods Forty-three patients with chronic angina pectoris undergoing coronary stent implantation received echocardiography before and 12 months after operation.Dynamic blood pressure was recorded after operation.According to hs-CRP levels detected before operation,the patients were divided into low hs-CRP group(hs-CRP≤3 mg/L,n=20) and high hs-CRP group(hs-CRP>3 mg/L,n=23).The left ventricular end-diastolic dimension,left ventricular end-diastolic volume,left ventricular end-systolic volume,left ventricular ejection fraction,hs-CRP and blood pressure variability were recorded.Results The level of hs-CRP was positively correlated with blood pressure variability.Compared with before treatment,all values of LVEDV,LVESV and LVEF increased(P<0.05),while left ventricular end-diastolic dimension only increased in high hs-CRP group(P<0.05).Conclusion The level of hs-CRP is positively correlated with blood pressure variability,and correlated with ventricular remodeling in the patients with stable angina pectoris after coronary stent implantation.
