幽门螺杆菌的基因型与抗生素疗效的关系

目的：分析幽门螺杆菌的ｃａｇＡ及ｖａｃＡ状态与抗生素疗效的关系。方法：广州地区１２６例让螺杆菌感染的消化性溃疡或功能性消化不良患者应用含克拉霉素的三联方案进行一周治疗，在治疗前胃镜采取胃窦活检标本进行幽门螺杆菌培养。并用ＰＣＲ检测ｃａｇＡ及ｖａｃＡ状态，在治疗结束４周后进行＾１３Ｃ呼气试验检测。结果：消化性溃疡与ｃａｇＡ阳性幽门螺杆菌感染密切相关，ｃａｇＡ及ｖａｃＡ状态对含克拉霉素的三联一周抗幽门螺杆菌治疗效果的无明显的影响。结论：幽门螺杆菌的ｃａｇＡ状态与消化性溃疡密切相关。但是ｃａｇＡ及ｖａｃＡ状态对抗生素疗效无明显影响。

Relationship between Genotype of H. Pylori and therapy Efficacy of Antibiotic

Aim: To determine the relationship between cagA and vacA status of H. Pylori isolated from clinic and the efficacy of antibiotic therapy. Methods: 126 patients with peptic ulcer and infected by H. Pylori of functional dyspepsia in Guangzhou region were treated with triple regimen containing olarithormycin for one week. H. Pylori was cultured from gastric antral biopsy specimens which were taken before treatment, and the cagA and vacA status was determined by polymerase chain reaction. Eradication of H. Pylori was assessed by 13C-urea breath test 4 weeks after the last medication was taken for 4 weeks. Results: Peptic ulcer was stongly associated with the cagA positivel H. Pylory infection, hadn' t significantly effect of the cagA and vacA status on the efficacy of anti-H. Pylori therapy with triple regimen containing clarithomycin for one week. Conclusions: The cagA status of H. Pylori was strongly associated with peptic ulcer, but there was no significantly effect of the cagA and vacA status on the therapy efficacy of antibiotic.
