三腔导尿管在危重腹泻患者肛周皮肤护理中的应用

目的探讨三腔导尿管在危重腹泻患者肛周皮肤护理中的应用效果。方法将60例腹泻危重患者分为治疗组和对照组,每组各30例。治疗组采用前端剪口改良后的28号三腔导尿管引流粪便,对照组采用传统护理方法,观察每组患者肛周皮肤红肿、糜烂等并发症。结果治疗组30例,26例皮肤完好,3例皮肤红肿,1例肛周糜烂;对照组30例,15例皮肤完好,9例皮肤红肿,6例肛周糜烂,两组皮肤完好率比较,χ2=7.5,P〈0.05,治疗组肛周皮肤治疗有效率优于对照组。结论采用三腔导尿管插入腹泻或大便失禁危重患者的肛门引流粪便能有效减少肛周皮肤损害,值得临床推广。

Application of three-cavity urethral catheter for draining sedes in perianal skin care of ICU patients with diarrhea

Objective To investigate the effect of three-cavity urethral catheter for draining sedes on protection of peri-anal skin of ICU patients with diarrhea.Methods 60 patients with diarrhea or fecal incontinence were equally randomized into the treatment and control group：the former received sedes draining by type 28 three-cavity urethral catheter and the latter by traditional nursing methods.The two groups were both observed in terms of perianal skin eczema,ulcers and inflammations.Results In the former group,the peri-anal skin of 26 patients was intact,that of 3 swelled and that of 1 patient presented anabrosis.In the latter group,the peri-anal skin of 15 patients was intact,that of 9 swelled and that of 6 patients presented anabrosis.In comparison of the rate of intact skin,the curative effect of the treatment group was significantly better than that of the control group（X^2 = 7.5,P〈 0.05）.Conclusion The type of three-cavity urethral catheter may effectively reduce lesions in perianal skin of ICU patients with diarrhea.
