血清胱抑素C对高血压早期肾损害的诊断价值

摘    要:目的探讨血清胱抑素C(CysC)对高血压患者早期肾损害的诊断价值。方法采用散射免疫比浊法测定65例高血压患者与58例健康对照者的血清CysC,苦味酸法测定血清肌酐(Scr),尿素酶法测定血清尿素(U-rea),并进行相关分析。结果高血压患者的血清CysC水平分别为:Ⅰ型高血压(1.06±0.24)mg/L,Ⅱ型高血压(1.72±0.89)mg/L,Ⅲ型高血压(2.27±1.32)mg/L;健康对照组血清CysC水平(0.88±0.17)mg/L。高血压组与健康对照组比较,差异有统计学意义(P0.01)。3型高血压患者CysC异常检出率分别为13.04%、52.63%、73.91%,Urea异常检出率分别为0.00%、15.79%、13.04%,Scr异常检出率分别为4.35%、10.53%、26.09%,Cy-sC异常检出率高于血清Urea和Scr。结论血清CysC是反映高血压早期肾功能损害的敏感指标,其敏感性高于Scr和Urea,在诊断高血压早期肾损害方面有重要的临床价值。

Abstract:Objective To investigate the diagnostic value of serum cystalin C in hypertensive patients with early renal damage. Methods To detect serum CysC by nepheiometry immunoassay,serum creatinine(Scr) by pieric acid method and serum urea(Urea) by urea enzymatic method in 65 cases of hypertension and 58 normal controls. The correlation analysis was performed. Results The serum levels of CysC in three types of hypertensive patients were: (1.06±0.2,t)mg/I. for type I , (1.72±0.89)mg/L for type II, (2. 27± 1.32)mg/L for type III ; in normal control group, serum level of CysC was(0.88 ± 0.17)mg/L. The difference had statistical significance comparing the hypertension group and the control group (P〈0.01). The abnormal detection rates in three types of hypertnsion were 13.04%,52.63% ,73.91% for CysC.0.00% ,15.79%,13.04% for Urea,4.35%,10.53% ,26.09% for Scr respectively. The abnormal detection rate of CysC was higher than that of serum urea and creatinine. Conclusion Ser um CysC is a sensitive indicator reflecting early renal damage of hypertension,its sensitivity is far superior to Scr and Urea,which has important clinical value in the diagnosis of early renal damage in hypertension.
