顺铂与香菇多糖胸腔灌注治疗恶性胸腔积液临床观察

目的:观察顺铂与香菇多糖胸腔灌注治疗恶性胸腔积液的疗效与毒副反应的差异。方法:28例患者随机分为A、B两组,予以胸腔内注入顺铂或香菇多糖,每周1次,连续2～3周。注射药物后观察两组治疗前后胸腔积液控制情况,平均生存期,生活质量改善及毒副反应情况。结果:A、B两组总有效率分别64.3%和78.5%(P>0.05),胸腔积液细胞学阴转率分别为50.0%和64.3%(P>0.05),有效病例胸腔积液平均缓解期和平均生存期分别为5.2个月和6.0个月(P>0.05),6.0个月和6.5个月(P>0.05)。分别有78.0%和85.0%的患者提高了生活质量(P>0.05)。A和B两组毒性反应轻微,均可耐受。结论:顺铂与香菇多糖胸腔灌注治疗恶性胸腔积液疗效均显著,可明显改善患者生活质量,且毒性反应轻微,可耐受。

Observation in the treatment of malignant pleural effusion by injection to cavity with lentinan,cis-piatinum

Objective:To observe clinical therapeutic effectiveness and adverse reaction in the treatment of malignant pleural effusion by injection to cavity with DDP or lentinan.Methods:Twenty-eight patients with malignant pleural effusion were divided into two groups:in the group A,DDP were used,while in the group B lentinan,was injected.Both were given once a week for consecutive 2～3 weeks.Therapeutic effect,progression-free and overall survival,life quality and adverse reactions were observed.Results:The overall objective responded rate(complete and partial remission)was 64.3% and 78.5%(P>0.05) respective.The negative rate of cyto-pathology was 50.0% and 64.3% respective.The average remission and progression-free and overall survival times in remission patients was 5.2 and 6.0 month(P>0.05).There were 78.0% and 85.0% patients in the two groups respective proved their life quality.The incidence of adverse reactions were lower in both two groups.Conclusion:Both DDP and lentinan are effective for malignant pleural effusion with mild side effects.
