儿童颅咽管瘤的显微手术疗效分析

目的探讨儿童颅咽管瘤在显微直视下手术全切的可行性。方法采用显微外科手术治疗儿童颅咽管瘤51例。年龄15岁以下（平均年龄9.2岁）。头痛、内分泌及视力障碍为主要临床症状。结果39例获手术全切,12例获手术次全切（其中6例为术后复发再手术）。3例术后死亡,39例获随访0.5～11年。肿瘤复发6例（平均术后24.6个月复发）。结论在熟练的显微直视下选择合适的手术入路行儿童期颅咽管瘤全切是手术成功的关键。本组资料进一步表明,首次手术全切肿瘤者效果最好。

Analysis of Curative Effect of Microsurgery on Craniopharyngiomas in Children

Objective To explore the possibility of total resection the children craniopharyngiomas under micresurgical orthophoria. Methods The retrospective analysis was performed in the 51 children with craniopharyngiomas treated by microsurgical technique. The age is under 15 year old（average 9.2 yearold）.The main symptoms are headache,visiual disorder and dyscrinism.Results Total removal was performed in 39 patients.Subtotal removal was performed in 12 patients（6 patients were operated after recurrent.）3 patients died after operation.The follow-up in 39 cases ranged from 0.5-11 years.6 eases suffered recurrent. （average 24.6 months after surgery）.Conclusion The selection of proper surgical approach is the key to total removal the children craniopharyngiomas under skillful micresurgery orthophoria.The material shows that the best effect is total resection the tumor at the first time.
