重症肌无力胸腺中CD4+CD25+ Treg细胞分布与数量变化的临床意义

摘    要:目的 探讨重症肌无力(myasthenia gravis, MG)患者胸腺内CD4 CD25 天然调节性T细胞(nTreg)的分布和数量变化及其临床意义.方法 采用免疫组化染色观察50例MG胸腺和20例正常胸腺CD25的表达, 同时采用流式细胞技术检测10例MG患者胸腺与外周血中CD4 CD25 T细胞在CD4 T细胞中的比例.结果 MG增生胸腺中CD25表达与对照胸腺相比显著升高(P<0.05),与MGFA分期呈正相关.增生胸腺中CD4 CD25 细胞占CD4 细胞比例明显高于对照胸腺(P<0.05),而外周血中CD4 CD25 T细胞比例无显著变化(P>0.05).结论 胸腺内nTreg细胞数量的下降与MG自身耐受的破坏有重要的关系,进一步研究胸腺内nTreg细胞数量和功能的变化有助于了解MG的发病机制.

Abstract:Objective To investigate the frequency of intrathymic CD4+ CD25+ regulatory T cells(nTreg)and its clinical significance in myasthenia gravis(MG)patients.Methods The intrathymic expression of CD25 in 50 MG patients and 20 normal thymus tissues as controls were analysed by immunohistochemical method and the frequency of CD4+CD25+ T cells derived from MG thymus and peripheral blood monocytes were detected in 10 MG patients by flow cytometry. Results The expression of CD25 in the hyperplastic thymus was much higher(P<0.05)than that in the control,and correlated with MGFA classification.The percentage of CD4+CD25+ T cells in MG hyperplastic thymus was higher than that in control(P<0.05),while the percentage in the peripheral blood monocytes was of no significant difference with the control.Conclusion The increased frequency of intrathymic CD4+CD25+ regulatory T cells has an important relationship with the destruction of self-tolerance in myasthenia gravis patients.Further investigation on the changes of frequency and function of the intrathymic nTreg cells would conduce to understand the mechanism of MG.
