纤支镜和X线联合导向下镍钛合金支架治疗重症恶性气道狭窄

目的评估纤支镜和X线联合导向下放置国产镍钛记忆合金支架治疗重症恶性气道狭窄的安全性及价值。方法12例重症恶性气道狭窄患者,气促评分3级3例,4级9例,在纤支镜和X线联合导向下放置国产镍钛记忆合金支架。结果放置1枚支架者11例,1例放置2枚支架。气促评分5例降为0级,7例降为1级,无严重并发症发生。术后3例接受放疗加静脉化疗,5例单纯放疗,1例单纯静脉化疗。随访1月~4年,除2例再次气道阻塞死亡外余未再出现呼吸困难。结论经纤支镜和X线联合导向下放置镍钛记忆合金支架具有操作简便、迅速、安全的特点,是治疗重症恶性气道狭窄的理想方法。

The Treatment of Malignant Serious Airway Stenosis using Nitinol Alloy Stent Implantation by Fibrobronchoscopy and under X-ray Guidance

Objective To evaluate the safety and effects of nitinol alloy stent made in China to treat malignant serious airway stenosis by fibrobronchoscopy and under X-ray guidance.Methods 12 patients with malignant serious airway stenosis were treated with nitinol alloy stents by fibrobronchoscopy and under X-ray guidance.The dyspnea indexes were 3 grade in 3 patients and 4 grade in 9 patients.Results One Stent was implanted into trachea in 11 cases and 1 patient was used 2 stents.The procedure was successful in all patients.Dyspnea was improved immediately.Dyspnea index:0 grade was in 5 patients and 1 grade was in 7 patients.No serious complications happened.3 patients accepted actinotherapy and chemotherapy,5 patients underwent actinotherapy only and one patient underwent chemotherapy.All patients were followed-up for one month to 4 years and no dyspnea happened except 2 patients.Conclusion Nitinol alloy stent implantation by fibrobronchoscopy and under X-ray guidance is simple,prompt and safe.It is a effective method in treating serious malignant airway stenosis.
