妊娠梅毒患者母婴传播的检测及干预治疗

目的 探讨妊娠梅毒检出率及干预治疗对阻断母婴传播的效果.方法 对孕妇及新生儿进行快速血浆反应素环状卡片试验(RPR)及梅毒密螺旋体血凝试验(TPPA)确诊检查,对妊娠梅毒患者及其分娩的新生儿进行干预治疗并随访观察.结果 在5194例孕妇中,RPR与TPPA均阳性117例,妊娠梅毒检出率为2.25％；在5 078例新生儿中,新生儿梅毒检出率为0.91％；孕妇治疗干预与新生儿脐带血梅毒快速血浆反应素(RPR)滴度的关系密切；新生儿脐带血RPR滴度随孕母RPR滴度升高而升高.结论 早期诊断妊娠梅毒是阻断、改善妊娠梅毒结局和降低先天梅毒发生率的关键,应加强孕前及孕期的梅毒检测及综合干预治疗,有效阻断母婴垂直传播.

Detection and Intervention Therapy of Maternal - infant Transmission of Pregnancy Syphilis

Objective To investigate the detection rate of the pregnancy syphilis and the effect of comprehensive intervention in maternal-infant transmission block of pregnancy syphilis.Methods The rapid plasma reagin circle card test(RPR)and Treponema pallidum particle assay(TPHA)were used to detect syphilis antibodies of pregnancy and their neonates.The pregnancy with syphilis and their neonates underwent the intervention treatment,and were followed up.Results Among the 5 194 lying-in women,the detection results of 117 specimens were RPR and TPHA positive,the detection rate of syphilis was 2.25%.Among the 5 078 infants,the detection rate of syphilis was 0.91%.The titer of RPR is connection to the comprehensive intervention of pregnancy syphilis.The rate of RPR titer of the newborn congenital syphilis was elected while the titer of their mother was high.Conclusion Early diagnosis is the key to block,improve pregnancy syphilis pregnancy outcome and reduce congenital syphilis incidence.We should strengthen the examination of pregnancy syphilis and comprehensive intervention,so can we effectively block the maternal-infant transmission.
