摘    要:目的 检测糖尿病肾病患者随机尿中的尿微量白蛋白(MA)/肌酐(Cr)比值、α1-微球蛋白(A1M)/肌酐(Cr)比值、尿免疫球蛋白(IgGU)/肌酐(Cr)比值和尿转铁蛋白(TRFU)/肌酐(Cr)比值,观察糖尿病肾病早期尿中微量蛋白变化,探讨其临床意义.方法 采用速率散射比浊法测定尿中的MA、A1M、IgGU、TRFU含量,利用肌酐比色法苯-苯反应测肌酐浓度,计算70例糖尿病肾病病人和40例健康人的尿白蛋白/肌酐比值、尿α1-微球蛋白(A1M)/肌酐、尿免疫球蛋白(IgU)/肌酐(Cr)比值和尿转铁蛋白(TRFU)/肌酐(Cr)比值.结果 尿白蛋白/肌酐比值与病情呈显著相关性,其灵敏性及特异性均较高.结论 四种微量蛋白/肌酐比值对诊断早期糖尿病肾病的一项敏感而可靠的指标.

Abstract:Objective To detect random urine micro - albu rain ( MA )/creatinine ratio ( Cr ), al - microglobulin ( AIM )/creatinine ratio, Immune globulins(IgGU)/creatinine ratio and transferrin(TRFU)/creatinine ratio in diabetic nephropathy patients, to observe urine micro- protein change in early phase of diabetic nephropathy and discuss their clinical significance. Methods Urine MA,AIM,IgGU and TRFU level were determined by rate nephelometry assay method, creatinine level were determined by benzene - benzene reaction creafinine colorimetric method, to calculate urine albumin/ereatinine ratio, al - mieroglobulin/creatinine ratio, Immune globulins/creatinine ratio and transferrin/ creatinine ratio in 70 diabetic nephropathy patients and 40 healthy control. Results urine albumin/creatinine ratio has significantly reiativity with patient's condition, it's sensitivity and specificity were high. Conclusion four micro - protein/creatinine ratio are sensitive and reliable marker for early diabetic nephropathy diagnosis.
