耻骨上前列腺摘除术并发症的原因及防治

研究目的探讨耻骨上前列腺摘除术后并发症的原因及防治措施。研究背景耻骨上前列腺摘除术是常用的术式,我院35年来采用该术式行前列腺摘除300余例,出现并发症33例。为减少并发症的发生探索其原因,提出防治措施。研究方法病例统计分析结果本纽33例的并发症有排尿困难,膀胱内出血,造瘘口漏尿,尿失禁,切口感染,急性附睾炎等,多由医源性因素引起,如用丝线缝合膀胱内口、术中止血不彻底,腺体残留,以及操作方法不当等。经过保守或再手术治疗,治愈32例,好转2例。结论并发症主要由医源性因素引起,术前、术中、术后均可发生。

The pathogenesis prevention and treatment of complications after suprapubic prospatectomy

Objective To search for the pathogenesise, prevention and treatment of complication after suprapubic prostatectomy.Background Suprapubic prostatectomy is a common kind of operation. About 300 cases of hypertrophy of prostate have been treated by this way for 35 years, among them 33 cases of complications appeared. In order to reduce the incidence of the complications, the pathogenesis, prevention and treatment are studied.Method Analysis on the cases statistics.Results In this group there were 33 patients with complications, such as dysuria, hemorrhage in bladder, suprapubic urinary fistula, incontinence of urine, wound infection, acute epididymitis, ect. They were caused by factors of treament itself. For example, suturing bladder neck with thread, stopping bleading incomplete in operation; prostate incompletely removed and unsuitable performance ect. With medical treatment and re-operation, 31 cases were cured, 2 cases were improved.Concelusion The complications resulted from iatrogenic factors and they may be taken place before, during and after operation.
