理疗床和手法治疗肩关节周围炎的随机对照临床观察

目的评价两种不同方法治疗肩关节周围炎的临床疗效.方法采用随机数字表将患者随机分为治疗组和对照组.治疗组36例,男8例,女28例,年龄45～64岁;对照组36例,男15例,女21例,年龄45～62岁.治疗组采用温热理疗床治疗4周.对照组采用推拿按摩手法治疗4周.分别在筛选期(治疗前)及治疗后14、28 d(治疗结束时)各观察记录1次,对比两组肩部疼痛症状、肩关节功能及症状疗效的改善.结果两组治疗前后各观察指标比较,肩部疼痛症状明显改善(P＜0.05);肩关节上举外展、外旋、后伸内旋活动角度均有明显增大(P＜0.05).治疗结果显示治疗组显效4例,有效25例,无效7例;对照组痊愈12例,显效13例,有效7例,无效4例.两组疗效比较差异有统计学意义(P＜0.05),对照组优于治疗组.结论理疗床和手法对肩关节周围炎引起的一系列临床症状及功能障碍均具有一定程度的改善效果.

Randomized control clinical observation on periarthritis of shoulder treated by physiotherapy couch and manipulations

Objective: To evaluate the therapeutic effect of periarthritis of shoulder treated by two kinds of physiotherapy. Methods: Patients were divided into treatment group and control group according to the random digits table. 36 patients were involved in treatment group, 8 male, 28 female; the average age was 54.5 years (range, 45-64 years). 36 patients were involved in control group, 15 male, 21 female; the average age was 53.5 years (range, 45-62 years). Patients in both groups were separately treated for 4 weeks, with lukewarm physiotherapy couch in treatment group, with massage manipulations in control group. The changes of pain and functions of shoulder were observed and recorded respectively before,during and after the therapy, and the results obtained from two groups were then compared with each other.Results: The pain and movements of shoulder in nearly all the directions in both groups were obviously improved (P<0.05).In treatment group, 4 patients gained excellent result, 25 valid, 7 invalid; in control group, 12 patients gained full recovery, 13 excellent result, 7 valid, 4 invalid. The results of two groups indicated significant difference statistically (P<0.05). However,the therapeutic effect of control group was better than that of treatment group.Conclusion: Both physiotherapy couch and manipulation can partially improve the clinical symptoms and function of periarthritis of shoulder.
