肝移植ICU鲍曼不动杆菌的感染特点及护理探索

【】 目的 探讨肝移植ICU病房下呼吸道感染患者鲍曼不动杆菌的感染特点,及相关护理干预措施对控制鲍曼不动杆菌感染的作用。方法 总结和分析肝移植重症监护病房2011年3-2012年3月17例鲍曼不动杆菌感染者的临床资料、感染原因及耐药特性,总结相应的护理干预措施。结果 药敏试验结果显示分离出的菌株对常用抗菌药呈现出多重耐药的现象。其中对氨基糖苷类(庆大霉素)、喹诺酮类(环丙沙星)、青霉素类(哌拉西林)、头孢类(头孢他啶、头孢噻肟、头孢吡肟)、碳青霉烯类(亚胺培南、美罗培南)耐药率均达到100%。通过临床资料分析显示,鲍曼不动杆菌感染的肝移植受体术后死亡率高,肝癌合并肝硬化患者,术前应用广谱抗生素(碳青霉烯类为主)、术中失血量大(≥3000ml)、呼吸机支持时间长(≥72h)、ICU滞留时间长、术后长期使用免疫抑制剂等都是造成感染死亡率高的危险因素。结论 肝移植ICU病房鲍曼不动杆菌耐药性强,肝移植病员术前及术后大量使用抗生素,术后常规使用免疫抑制剂,在ICU的术后过渡期等原因会发生鲍曼不动杆菌院内获得性感染,治疗较为棘手,因此在使用敏感抗菌药物治疗同时,必须对患者采取相应护理措施,加强医院感染管理力度,做好隔离措施,以促进患者恢复,避免引发交叉感染,保护其他易感患者。

Infection characteristics of Acinetobacter baumannii and nursing interventions of related infections in liver transplantation ICU

Objective To investigate the drug resistance of Acinetobacter baumannii(AB) in patients with lower respiratory tract infection in liver transplantation ICU ward,and to explore the effect of related nursing interventions. Methods 17 strains resistant characteristics AB were separated from the sputum samples collected from 17 patients with the lower respiratory tract infection in liver transplantation ICU and the differences in clinical data , cause of infection, drug resistance were compared. The appropriate nursing interventions was summed up.
