肾组织中足细胞标志物synaptopodin的表达与肾脏损害关系的临床意义

目的研究足细胞标志物synaptopodin和WT-1在肾病患者中的表达变化，探讨其对肾脏病尤其是在糖尿病肾痛患者中进展的影响。方法应用间接免疫荧光双标记和激光共聚焦显微镜技术检测正常对照组及不同病变程度糖尿病肾病患者肾组织内synaptopodin和WT-1的表达。结果与正常对照组相比，足细胞数量、synaptopodin和WT-1的表达在糖尿病肾病患者肾组织内表达显著减少（P〈0．05）；结节性硬化组较弥漫性系膜硬化组减少更为显著（P〈0．05）。结论糖尿痛肾痛患者·肾小球内wnaptopodin表达降低一方面是足细胞数量减少所致，另一方面其表达降低又可促进肾小球损伤，减轻足细胞的损伤将成为延缓糖尿病肾病进展的重要措施。

Treatise Clinical significance of the relationship between synaptopodin manifestation of Sertoli cell marker in nephridial tissue and kidney damage

Objective To research into the manifestation changes of synaptopodin of Sertoli cell marker and WT-1 in patients with kidney disease and to probe into its influence on patients with kidney disease and particularly with diabetic nephropathy.Methods Indirect immunofluorescence double-tagging and laser scanning confocal microscope technique were adopted to detect the manifestation of synaptopodin and WT-1 of nephridial tissue in the control group patients and the patients with different lesion level diabetic nephropathy.Results Compared with those of the control group,the Sertoli cell number and the manifestation of synaptopodin and WT-1 of nephridial tissue in the patients with diabetic nephropathy remarkably decreased(P<0.05).And they decreased even more greatly in the tuberous sclerosis group than in the diffuse mesangial sclerosis group(P<0.05).Conclusion The decrease of the manifestation of synaptopodin of glomerulus in the patients with diabetic nephropathy resulted from the decrease of the Sertoli cell number,and meanwhile the decrease of the manifestation can also increase the glomerulus damage.Reducing Sertoli cell damage will be an important measure to delay the progress of diabetic nephropathy.
