Th1/Th2炎性细胞因子在老年肺气肿与肺间质纤维化中的研究

目的 探讨在中国汉族老年人群中,肺气肿和肺间质纤维化患者血Th1/Th2炎性细胞因子的浓度差异趋势.方法 选取肺气肿、肺间质纤维化老年患者和健康人各40例,采集外周静脉血5 ml,采用酶联免疫吸附法检测血清Th1/Th2炎性细胞因子水平,病例组与健康对照组的比较采用独立样本t检验,Th1/Th2炎性细胞因子之间的相关关系采用Pearson直线相关分析.结果 3组平均年龄分别为:健康人组58.26±9.21岁、肺气肿组60.26±10.43岁、肺间质纤维化组61.26±8.83岁,差异均无显著性(P＞0.05).在肺气肿组中干扰素γ(INF-γ)、白介素(IL)-2水平明显升高P ＜0.01,而Th2炎性细胞因子IL-4水平显著降低(P＜0.01),差异具有显著的统计学意义；在肺间质纤维化中,IL-2水平明显降低(P＜0.01),而IL-5、IL-10水平则明显升高(P＜0.05),差异具有统计学意义；同时结果显示细胞因子间存在正相关关系.结论 在老年肺气肿和肺间质纤维化患者Th1/Th2炎性细胞因子有显著的浓度差异变化,其结果为临床治疗提供一个新的视点与思路.

Study on changes of Th1/Th2 inflammatory cytokines in elder patients with emphysema or interstitial pulmonary fibrosis

Objective To explore changes in concentration of Th1/Th2 inflammatory cytokines in elder patients with emphysema or interstitial pulmonary fibrosis in Chinese Han population.Methods This study was performed in 40 cases of emphysema and 40 cases of interstitial pulmonary fibrosis in elder patients.Peripheral blood samples with 5 ml each were collected from every individual and serum levels of Th1/Th2 inflammatory cytokines were detected by ELISA.Results Of serum levels in patients of study groups and healthy control group were compared by using independent sample t-test and the correlation between Th1/Th2 inflammatory cytokines was tested by using the Pearson linear correlation analysis.Results The average age ranges of these three groups were 58.26 ± 9.21 years in healthy persons,60.26 ± 10.43 years in emphysema group,and 61.26 ± 8.83 years in pulmonary fibrosis group.The difference among these 3 groups was not statistically significant（P0.05）.In patients of emphysema group,INF-gamma and IL-2 levels were significantly higher,but Th2 inflammatory cytokine IL-4 level was significantly lower,and their difference was statistically significant（P0.01）.There was statistically significant difference between interstitial pulmonary fibrosis group and control group,IL-2 level was significantly lower（P0.01）,and IL-5 and IL-10 levels were significantly increased（P0.05）.At the same time,positive correlation was exhibited in all cytokines.Conclusion Th1/Th2 inflammatory cytokines possess a significant concentration difference in elder patients with emphysema and elder patients with interstitial pulmonary fibrosis.The result of this study will provide a new point of view in clinical treatment of patients with these two kinds of diseases.
