鞍区肿瘤MRI诊断及鉴别诊断探讨

目的 总结并探讨鞍区肿瘤MRI诊断及鉴别诊断特点,评价应用价值.方法 回顾性分析2006年3月至2009年2月经病理学检查确诊鞍区肿瘤患者52例MRI影像学资料.结果 52例患者中通过病理检查或外科手术确诊垂体腺瘤19例,脑膜瘤9例,颅咽管瘤7例,生殖细胞瘤6例,三叉神经瘤4例,胶质瘤4例,畸胎瘤2例,脊索瘤1例.MRI诊断与外科手术或病理检查确诊符合率达到98.1％ (51/52),1例病人颅咽管癌被误诊为垂体腺瘤.不同类型鞍区肿瘤均有各自好发部位,年龄阶段,且具有独特MRI影像学表现.结论 MRI影像学资料能够有效辅助鞍区肿瘤临床诊断及鉴别诊断,具有重要推广使用价值.

Study on the significance of MRI in diagnosis and differential diagnosis of sellar tumors

Objective To summarize and discuss the significance of MRI in diagnosis and differential diagnosis of sellar tumors.Methods The data of MRI in 52 cases of sellar tumors including 20 cases of pituitary adenoma,9 cases of meningioma,7 cases of craniopharyngioma,6 cases of germinoma,4 cases of trigeminal neuroma,4 cases of glioma,2 cases of teratoma and 1 case of chordoma in this hospital during March 2006 to February 2009 were retrospectively analyzed.Results In this study,these 52 patients through pathological examination or surgical operation were confirmed the diagnosis of pituitary adenoma in 19 cases,meningioma in 9 cases,craniopharyngioma in 7 cases,germ cell tumor in 6 cases,trigeminal neuroma in 4 cases,glioma in 4 cases,teratoma in 2 cases and chordoma in 1 case.The accordance rate of MRI with surgical operation or pathological examination was 98.1%（51/52）,1 patient with craniopharyngioma had been misdiagnosed as pituitary adenoma.The different kinds of sellar tumors were all possess their different predilection sites,age of onset and unique MRI findings.Conclusion The data of MRI are helpful in supplementally improving the diagnosis and differential diagnosis of sellar tumors and MRI is worthy to be recommended for application in clinical areas.
