268例肺结核患者耐药菌谱分析

[目的]检测和分析结核分枝杆菌（MTB）的耐药性，为临床诊断与治疗提供实验依据。[方法]分离我院结核三科2009年10月-2010年10月住院结核患者痰标本中MTB阳性，并进行异烟肼、利福平、链霉素和乙胺丁醇4种抗结核一线药物的敏感试验。[结果]268例肺结核患者（初治194例，复治74例），在所有病例中，分离的MTB对4种抗结核药物均敏感的187例（69．8％187／279），存在不同类别耐药的81例（30．2％81／268）。其中，单耐药在初治病例中多见，耐多药和多耐药均在复治病人中比较高。[结论]应加强结核病患者痰MTB的检测，根据药敏结果分层次制定不同方案，以提高治愈率，减少耐药结核病尤其是耐多药结核病例的产生。

ANALYSIS OF DRUG- RESISTANT SPECTRUM IN 268 TUBERCULOSIS PATIENTS

[ Objective ] To detect and analyze the Mycobacterium tuberculosis （MTB） drug resistance to provide the experimental evidence for clinical diagnosis and therapy. [ Methods] Tuberculosis was isolated in our hospital from October 2009 to October 2010 in sputum of patients hospitalized tuberculosis MTB positive, and for isoniazid, rifampicin, streptomycin and ethambutol 4 first - line anti - TB drugs sensitive test. [ Results] 268 cases of tuberculosis （ 194 cases of initial treatment, re - treated 74 cases） were found. For all the 268 cases, 4 kinds of MTB isolated were sensitive to anti - TB drugs to 187 patients （69.8%, 187/279）. There were different types of resistance in 81 patients （30.2% 81/268）. Among them, the single drug common in untreated cases of MDR and multi - drug resistance among re -treatment were relatively high. [ Conclusion] MTB sputum should be closely monitored in TB patients. Antibiotics should be reasonably used according to the results of the drug sensitive test.
