摘    要:目的探讨非糖尿病慢性肾脏病(CKD)患者血尿酸(Uric Acid,UA)与估计肾小球滤过率(eGFR)是否存在性别差异,为临床针对不同性别患者采用不同强度的降低尿酸方案延缓肾脏病的进展提供参考。方法收集2010年10月-2011年7月期间宁夏医科大学总医院门诊及住院CKD 1~4期患者血尿素氮、血肌酐、血尿酸等相关资料,分析不同性别间血尿酸与eGFR相关关系,比较两组间相关关系差异。结果本研究共纳入CKD 1~4期患者235例,其中男120例,女115例,不同性别间年龄、BMI、平均动脉压、合并高血压病、血尿素氮、白细胞计数、血红蛋白、CRP、血钙、血磷、血清白蛋白、血清总蛋白、LDL、HDL、TG、24h尿蛋白定量等可能影响eGFR的因素差异均无统计学意义(P>0.05)。CKD 1~4期男、女性患者血尿酸水平与eGFR相关性分析均呈线性相关(男性r=-0.345,P<0.01;女性r=-0.462,P<0.01)。性别间eGFR/UA差别有统计学意义(P<0.05),女性比值较男性高。结论非糖尿病慢性肾脏病患者血尿酸和eGFR关系存在着性别差异,高尿酸对女性患者的eGFR影响更明显。

Abstract:Objective To explore relationship between eGFR and serum uric acid level in patients with chronic kidney disease in different genders.Methods CKD patients at stage1-4 were reviewed in general hospital of NingXia medical university between October,2010 and July,2011.Data including blood urea,serum creatinine,serum uric acid were collected.Linear regression was performed to analysize relationship between serum uric acid level and eGFR.Further analysis was performed between male and female.Results 235 patients with stage1-4 CKD including 120 males and 115 females were recruited in this study.Age,BMI,mean arterial blood pressure,hemoglobin,Serum creatinine,serum calcium,serum phosphorus,serum albumin,serum total protein showed no significant difference between males and females(P > 0.05).Linear re-gression indicated relevance between uric acid and eGFR were significant both in males and females(Male R =-0.345,P < 0.01;Female R =-0.462,P < 0.01).eGFR / UA in males was significantly higher than that in females(P < = 0.05).Conclusion Serum uric acid is the independent risk factors for non-diabetic CKD patients.The relatiship between eGFR and serum uric acid level in patients with chronic kidney disease showed sex difference.
