| 摘 要： | 目的探讨肝硬化患者中糖尿病与原发性肝癌（primary hepatic carcinoma，PHC）发生的相关性。方法对2001年12月-2008年12月解放军第jo二医院诊断为肝炎肝硬化、酒精性肝硬化患者22132例（合并PHC患者6169例为病例组）进行病例对照研究，分析肝硬化患者中糖尿病与PHC的发生有无相关性，并按病因（包括乙型肝炎肝硬化、丙型肝炎肝硬化和酒精性肝硬化）分层分析其相关程度。结果在肝硬化患者中，合并糖尿病患者发生PHC的OR值为1．851（95％CI，1．693～2．023，X^2=184．55，P=0．000），其中乙型肝炎肝硬化、内型肝炎肝硬化和酒精性肝硬化组的OR值分别为1．850（95％CI，1．666～2．050，X2=137．52，P=0．000）、2．238（95％CI，1．738～2．882，X2=38．97，P=-0．000）和1．935（95％CI，1．472～2．567，X^2=20．89，P=0．006）。结论肝硬化患者合并糖尿病可以增加发牛PHC的胍险，在丙型肝炎肝硬化患者最为明显。
