颅内动脉瘤电解可脱性弹簧圈栓塞后MRA随访

摘    要:目的评估磁共振血管造影(MRA)在动脉瘤弹簧圈栓塞后随访中的价值.方法回顾栓塞后3个月同期进行了MRA和数字减影血管造影(DSA)复查的37例41个动脉瘤,两者时间间隔在3d以内,以DSA为标准,观察有无瘤颈残留以及弹簧圈内有无血流残留.结果 41个动脉瘤中,DSA发现有29个完全闭塞,9个可见瘤颈残留,3个动脉瘤内有对比剂.MRA见31个动脉瘤完全闭塞,8个可见瘤颈残留,2个可见瘤内存在血流.本组中假阴性2例,无假阳性.结论 MRA是脑动脉瘤弹簧圈栓塞后的一种无创、可靠、快速的影像学随访方法,有助于监测动脉瘤颈残留和弹簧圈内血流残留.

Abstract:Objective To evaluate the value of MR angiography (M RA) in the follow-up of intracranial aneurysms treated with electrolyticall y detachable coils.Methods Thirty-seven patients harboring 41 intracranial aneurysms previously treated with electrolytically detachable coils were included in this retrospective study. All the patients underwent both thre e-dimensional time-of-flight MRA and digital subtraction angiography (DSA) 3 months after endovascular therapy. The interval between MRA and DSA was less tha n 3 days. The goal was to evaluate MRA contrasting to DSA for the detection of a residual aneurysm neck or residual flow inside the coil mesh. DSA was consi dered as the gold standard for diagnosis.Results In the 41 aneu rysms, DSA revealed complete occlusion in 29 cases, presence of residual ane urysm neck in 9 cases, and presence of residual flow inside the coil mesh in 3 cases. MRA showed complete occlusion in 31, presence of residual aneurysm nec k in 8, and presence of residual flow inside the coil mesh in 2 aneurysms. MRA m issed a residual aneurysm neck and a residual flow inside the coil mesh. Conclusion MRA is a noninvasive, valuable and fast method for the follo w-up of coiled intracranial aneurysms. It is useful in the monitoring of residu al aneurysm neck or residual flow inside the coil mesh after treated with coils.
