36例老年肾功能衰竭患者血液透析疗效分析

本文就我院１９８７年１月至１９９６年５月收治的３６例老年肾衰患者血透的疗效进行回顾性分析，并与同期随机选择的２８例成年人（年龄＜６０岁）肾衰患者的血透疗效进行对比。老年肾衰患者透析前合并症高达４０％，以心血管系统疾病居多。成年组透析前合并症２５％。透析后全部患者尿素氮、肌酐均明显下降，慢性心功能不全及代谢性酸中毒得到不同程度改善，合并高血压者透析后血压水平明显下降。尽管老年患者血液透析危险性较大，但可避免因腹膜透析而产生的腹膜并发症，能及时纠正水、电解质及酸硷平衡失调。对老年肾功能衰竭患者。掌握恰当的时机，选择适合患者病情的透析时间、超滤率及出超量，能够改善病人预后，提高生存率。

Efficacy of hemodialysis in 36 elderly patieants with renal failure

The efficacy of hemodialysis in 36 old pationts with renal failure admitted from Jan 1978 to May 1996was analysed retrospectively and compared with that in 28 randomly selected adult patients in the same period as cohtrol. Before hemodialysis the incidence rate of complications was up to 40%in the aged,of which the cardiovascular complications occured frequently.but inthe abult group.the rate was only 25% After hemodialysis.blood urea nutrogen (BUN)and creatinine (cr) consideally decreased in both groups.Chronic heart failure and metabolic acidosis improved in some degree.Bolld pressures of patients with hypertensiom were remardedly lowered.Although hemodialysis has high risk in the aged,it may avoid most complications caused by peritoneal dialysis and restorewater,electrolytes and acid-base balance,Inelderly payients with real faiture Hemodialysis may improve prognosis and increase the surrival rate of elderly payients with real faiture.
