热灌注化疗治疗恶性胸腔积液的临床观察

目的观察体外高频热疗联合胸腔热灌注化疗治疗恶性胸腔积液的临床效果及不良反应。方法选择广东省罗定市人民医院38例已确诊的胸腔积液患者，并分为观察组与对照组，每组各19例。胸腔穿刺引流干净胸腔积液后，观察组给予热疗＋生理盐水1000mL＋顺铂80mg／m2，对照组则单纯给予生理盐水1000mL＋顺铂80mg／m2注入胸腔内。两组均每周灌注2次，连续3周，评定临床效果。结果观察组的近期总有效率为78．95％，KPs评分总提高率为63．16％；而对照组的近期总有效率为42．11％，KPS评分总提高率为26．32％。观察组的胸腔积液控制率、KPS评分升高率明显优于对照组（P〈0．05）。两组不良反应无明显差异。结论体外高频热疗联合胸腔热灌注化疗能较好地治疗恶性胸腔积液，且能提高晚期肿瘤患者的生活质量，副作用少，耐受性好，值得临床推广应用。

Clinical observation of perfusion chemotherapy in the treatment of malig- nant pleural effusion

Objective To observe the clinical effects and adverse reactions of the external high frequency hyperthermia combined with pleural perfusion chemotherapy in the treatment of the malignant pleural effusion. Methods 38 patients with pleural effusion have been diagnosed, and divided into observation group and control group, 19 cases in each group, all the patients had the thoracentesis to drainage clean the pleural effusion, the observation group were treated with hyperthermia combined with saline 1000 mL plus cisplatin 80 mg/m2 control group treated with saline 1000 mL plus eisplatin 80 mg/m2 injected into the pteural cavity. Both groups infusion 2 times a week for three consecutive weeks to assess the clinical effect. Results The total effective rate of observation group was 78.95%, the KPS score improve the rate of 63.16%; recent infusion chemotherapy with cisplatin alone total effective rate was 42.11%, KPS score increased rate was 26.32%. Pleural effusion control rate of the observation group, KPS score the elevated rate significantly better than that of the control group （P 〈 0.05）. No significant difference between two groups of adverse reactions. Conclusion The external high frequency thermotherapy joint pleural hot infusion chemotherapy for the treatment of malignant pleural effusion can also improve the quality of life of patients with advanced tumors, with few side ef- fects, well tolerated.
