正常血压高值与肾损害发生的相关性分析

目的观察正常血压高值人群肾功能的变化,探讨二者的相关性。方法将门诊体检者根据血压水平分为理想血压组86例、正常血压高值组67例与高血压组72例。采用放免法和酶法分别测定尿β2微球蛋白(β2-MG)、尿微量白蛋白(mAIb)及血尿素氮(BUN)、肌酐(Cr)水平。结果三组患者在年龄、体重指数、血糖、总胆固醇(TC)、总甘油三酯(TG)水平无明显差异,正常血压高值组尿β2-MG、mAIb及血Cr明显高于理想血压组(P0.05或P0.01),SBP、DBP与尿β2-MG、mAIb、血BUN、Cr均呈正相关,血压水平与尿β2-MG、mAIb有显著相关性(P0.01),血Cr与DBP及体重指数相关(P0.0`1)。结论正常血压高值人群可能存在与血压水平相关的早期肾损害。

Relationship between Prehypertension and Kidney Damage

Objective To observe the renal function in prehypertension and explore their correlation. Methods The volunteers were divided into normotension group （86 cases）,prehypertension group （67 patients） and hypertension group （72 cases） according to blood pressure levels. Urine β2 mieroglobulin（ β2-MG）,urinary albumin （mAIb） and blood urea nitrogen（BUN） and creatinine（Cr） level were measured by radi y and enzyme method,respectively. Results There were no significant differences in age, body mass index,blood glucose,total cholesterol （TC）and total triglyceride （TG） levels in the three groups. Urinary β2-MG, mAIb and blood Cr in the prehypertension group were significantly higher than those in the normotensive group （P〈0.05 or P〈0.01 ）. SBP and DBP were positively correlated with the urinary β2-MG,mAIb,serum BUN and Cr. The blood pressure level was positively correlated with the urinary β2-MG and mAIb（P〈0.01）,and the Blood Cr and DBP were positively correlated with the body mass index（P〈0.01）. Conclusion Early kidney damage related to the blood pressure level may exist in prehypertension.
