全葡萄膜炎的临床特征及诊治分析

目的 探讨全葡萄膜炎的临床特征及治疗.方法 回顾分析2011年8月至2013年6月我院眼科诊断为全葡萄膜炎并接受住院治疗患者的病例资料.结果 26例（32只眼）全葡萄膜炎患者中,男性17例,女性9例,男女比例为1.89：1,平均年龄为（40.1±12.1）岁.其中特发性10例（42.1%）,小柳原田综合征4例（15.4%）,交感性眼炎3例（11.5%）,病毒性3例（11.5%）,Behcet病2例（7.7%）,急性视网膜坏死综合征2例（7.7%）,强直性脊柱炎1例（3.85%）和尘肺伴肺结核1例（3.85%）.经局部糖皮质激素、非甾体药物、睫状肌麻痹剂,全身糖皮质激素甚至免疫抑制剂,以及抗病毒及抗结核,多数全葡萄膜炎患者视力得到改善.结论 全葡萄膜炎对视力损害严重,治疗周期较长,不及时、合理的治疗常常导致盲目的 发生,短期的住院治疗仅使患者炎症得以部分控制,炎症的彻底控制仍需较长时间的药物维持;同时全葡萄膜炎的病因复杂,给眼科医生正确诊断和治疗带来了困难.

Clinical manifestations and treatment of panuveitis

Objective To analyze characteristics of clinical manifestations and treatment options of panuveitis. Methods We retrospectively analyzed 26 cases of panuveitis. Patients were hospitalized in Taihe Hospital in Shiyan from August 2011 to January 2013. Results Among 26 patients, seventeen were male and 9 were female, and the gender ratio was 1.89：1. Average age was 40. 1 ±12. 1. Etiologies included idiopathic （ 10 cases, 42.1% ）, Vogt-Koyanagi-Harada syndrome （4 cases, 15.4%）, sympathetic ophthalmia （3 cases, 11.5%）, herpetic uvcitis （3 cases, 11.5%）, Behcet disease （2 eases, 7.7% ） , acute retinal necrosis syndrome （2 cases, 7.7% ） , ankylosing spondylitis （1 case, 3.85 % ） , and pneumoeoniosis complicated with tuberculosis （ 1 case, 3.85% ）. After treatment with corticosteroids, non-steroids drugs, cycloplcgic eye drops, immunosuppressant drugs, antiviral or anti-tuberculosis therapy, visual acuity improved in most patients. Conclusion Panuveitis impaired visual acuity severely, and needs a long course treatment. Untimely and inadequate treatment may lead to blindness. Short duration of treatment can only alleviate the inflammation and cannot control the disease completely. Complete control of panuvcitis needs a long course of adequate medication.
