含莫西沙星的补救方案对根除Hp的疗效分析

目的观察含莫西沙星的三联疗法对幽门螺旋杆菌（Hp）根除失败患者的补救治疗效果及根除率。方法人选者为经14C-呼气试验证实为Hp阳性患者127例，至少行幽门螺旋杆菌根除治疗1次以上，所有患者随机分成2组，莫西沙星三联组64例给予含莫西沙星的三联疗法（莫西沙星＋阿莫西林＋雷贝拉唑）14d；标准三联组63例给予标准三联疗法（克拉霉素＋阿莫西林＋雷贝拉唑）14d。2组治疗结束后，停药1个月后复查14C-呼气试验，比较2组患者根除幽门螺旋杆菌的疗效及根除率。结果莫西沙星三联组Hp根除率93．8％（60／64），标准三联组Hp根除率74．6％（47／63），2组Hp根除率比较，差异有统计学意义（P〈0．05），2组均无严重的副反应。结论对幽门螺旋杆菌根除失败患者，给予含莫西沙星的三联疗法比标准三联疗法有更好的治疗效果。

Efficacy analysis of patients failed for eradication of helicobacter pylori remedial treatment with moxifloxacin tri-pie therapy

Objective To evaluate the effect of remedial treatment with moxifloxacin triple therapy as remedial treatment of eradication of H. pylori infection. Methods A total of 127 patients with HP infection （ 14 C-urea breath test positive） failing in previous treatment one or more were enrolled and divided into 2 groups. The moxifloxaein based group was sim- ultaneously treated with amoxicillin and rabeprazole（ moxifloxacin 0.4 g po qd ＋ amoxicillin 0.5 g po bid ＋ rabeprazole 10 mg po bid, 14 d） ;the control group was treated with clarithromycin, amoxicillin and rabeprazole（ clarithromycin 0.5 g po bid ＋ amoxicillin O. 5 g po bid ＋ rabeprazolel0 mg po bid, 14 d）. The eradication rate of Hp was evaluated by 14C-urea breath test 4 weeks after the treatment. Results The Hp eradication rates of the moxifloxacin based group was 93.8% while that of the other group was 74.6% （ P 〈 0.05 ）. The difference of eradication rate was significant. No significant ad- verse reaction was observed. Conclusion The moxifloxacin based triple therapy was considered as one of effective thera- py options for the patients who failing in previous treatment.
