不同充填材料在老年人根面龋治疗中的应用效果比较

目的观察富士Ⅸ玻璃离子与3MZ250光固化复合树脂用于充填老年人根面龋的临床效果。方法选择2011年6月-2012年6月期间于口腔科门诊就诊的老年根面龋患者共272例，患牙392颗，根据患者就诊顺序分为A、B两组。2组患者行充填治疗前应首先对牙龈炎和牙周炎进行相应处理，待炎症控制后方可进行充填治疗。A组给予富士Ⅸ玻璃离子充填，B组给予3MZ250光固化复合树脂充填。随访观察两种充填材料的充填效果。结果充填半年内，富士Ⅸ玻璃离子的充填成功率为99．03％，光固化复合树脂为97．85％，x2=0．0182，P〉0．05，组间比较差异无统计学意义；充填一年，富士Ⅸ玻璃离子的充填成功率为94．17％，光固化复合树脂为88．17％，x2=11．915，P〈0．05，富士Ⅸ玻璃离子的充填成功率高于光固化复合树脂；充填一年半，富士Ⅸ玻璃离子的充填成功率为93．14％，光固化复合树脂为79．57％，x2=30．883，P〈0．05，富士Ⅸ玻璃离子的充填成功率高于光固化复合树脂。充填失败的病例中最常见的是充填体边缘不密合，其次是充填物发生缺损。A组中6例发生充填物边缘不密合，5例发生充填物缺损，3例发生充填物脱落，2例发生继发龋；B组中19例发生充填物边缘不密合，11例发生充填物缺损，7例发生充填物脱落，1例发生继发龋。结论富士Ⅸ玻璃离子与光固化复合树脂充填老年患者根面龋的近期效果无明显差异，但充填一年后富士Ⅸ玻璃离子的成功率明显高于光固化复合树脂，且玻璃离子的充填操作简单，治疗舒适性更好，值得临床推广。

Comparison of effect of different filling material applied on root caries in elderly

Objective To observe the effect of Fuji Ⅸ glass ionomer and 3 M Z250 light-cured composite resin for the root caries in elderly. Methods Total 392 teeth in 272 patients treated in our department from June 2011 to 2012 were select- ed and divided into A and B group by the order in treatment. Gingivitis and periodontitis should be treated before filling, and after the inflammation was controlled, the filling treatment was given. Fuji Ⅸ glass ionomer was applied in group A, while 3M Z250 light-cured composite resin applied in group B. The therapeutic effect of the materials was observed in follow-up. Results Half year after the filling,the success rate of Fuji Ⅸ glass ionomer was 99.03% ,while 3M Z250 light- cured composite resin was 97.85% , X2 = 0. 0182 ,P 〉 0.05, and the difference was not statistical significant between the two groups. One year after filling,the success rate of Fuji Ⅸ glass ionomer was 94.17% ,while 3M Z250 light-cured com- posite resin was 88.17 % , X2 = 11.915, P 〈 0.05, the rate of Fuji Ⅸ glass ionomer was higher than that of 3 M Z250 light- cured composite resin. One and half years after filling, the success rate of Fuji Ⅸ glass ionomer was 93.14% , while 3M Z250 light-cured composite resin was 9.57% , X2 = 30. 883, P 〈 O. 05, the rate of Fuji Ⅸ glass ionomer was higher than that of 3M Z250 light-cured composite resin. The most common situation in the cases of failure was poor adhesion at edge of the filling body,and the defect on filling was little. There were 6 cases of poor adhesion at edge of the filling body,5 ca- ses of defects on filling and 2 cases of secondary caries in group A,while 19 cases of poor adhesion at edge of the filling body, 11 cases of defects on filling and 1 case of secondary caries in group B. Conclusion There was no significant differences in therapeutic effect between Fuji Ⅸ glass ionomer and 3M Z250 light-cured composite resin on root caries in elderly,but one year after the filling,the success rate of Fuji Ⅸ glass ionomer is higher than that of 3M Z250 light-cured composite resin. And the operation of glass ionomer is simple, and treatment comfort is better. It is worthy to be popular- ized in clinical treatment.
