螺旋CT多平面重建技术对极外侧型腰椎间盘突出症的诊断

摘    要:目的 探讨螺旋CT多平面重建(MPR)技术对极外侧型腰椎间盘突出症(ELDH)的CT分型和特征表现。材料与方法 对11例经手术证实的ELDH的MPR表现进行分析。结果 11例ELDH与椎弓根的关系分为3型:椎间孔型6例,椎间孔外侧型2例,混合型3例,包括椎间孔内外侧型混合以及与椎管内后外型的混合。MPR显示ELDH的特征为椎间孔内软组织块影。MPR准确地提供了ELDH的解剖信息包括位置、形状和大小等。结论 MPR是诊断ELDH的可行方法,有助于手术方案的选择。

Abstract:Objective To study CT characteristics and classification of extremely lateral lumbar disc herniation (ELDH) with spiral CT multiplanar reformation (MPR).Materials and Methods MPR findings of 11 ELDH confirmed by surgery were analyzed.Results According to the relation of ELDH and pedicle of vertebral arch, ELHD could be classified into three types: intervertebral foraminal type (n=6), external intervertebral foraminal type (n=2) and mixed type (n=3). On MPR, ELDH was characterized as a soft tissue density mass in intervertebral foramen. The site, shape and size of ELDH could be well demonstrated on MPR.Conclusion MPR is an effective method in diagnosing ELDH and is very helpful for the selection of surgical management.
