摘    要:目的探讨经纤维支气管镜不同取材方法对肺癌的诊断价值。方法回顾分析钳检、刷检、灌洗及针吸活检4种不同取材方法对肺癌的检出情况。结果确诊的196例肺癌患者镜下分为四型:增生型、浸润型、外压型及正常型;钳检、刷检、灌洗及针吸活检的阳性率分别为70.7,62.2,54.8和51.7%。不同病变分别采用刷检联合其他3种检查的阳性率平均为77.9%。不同取材方法对不同镜下类型肺癌的检出不同。结论纤维支气管镜对肺癌的诊断有重要价值,4种取材方法中钳检的检出率最高;运用联合取材方法可提高肺癌的诊断率,不同取材方法对不同镜下类型肺癌的诊断价值不同。

Abstract:Objective:Assess the clinical value of different drawing materials methods through fibrobronchoscope in diagnosing lung cancer.Methods:To review the positive rate in detection lung cancer with different methods:forceps biopsy,brushing biopsy,douche and aspiration biopsy.Results:The manifestation in196final diagnosed lung cancer patients with fibrobronchoscope could be classified of four types:proliferation,infiltration,suppression and normal forms.The respective positive rate of forceps biopsy,brushing biopsy,douche and aspiration biopsy was70.7%,62.2%,54.8%and51.7%.The average positive rate in different lesion with brushing biopsy combining other three methods was77.9%.The positive rate is different when using different drawing materials methods through fibrobronchoscope in different types of lung cancer.Conclusions:Fibrobronchoscope is important in diagnosing lung cancer.Forceps biopsy's positive rate is the highest in four methods.Using associated methods can improve lung cancer's final diagnosing rate,different methods have different diagnosis value in different type of lung cancer.
