急性白血病病人网织血小板变化及其临床意义

目的 探讨急性白血病（AL）病人外周血网织血小板（RP）变化及其临床意义。方法 用流式细胞仪（FCM）检测32例不同类型AL病人的RP、血小板（PLT），动态观察7例病人化疗过程中的RP、PLT的变化，并与12例健康查体者（正常对照组）进行比较。结果 AL初治组、复发难治组RP、PLT低于正常对照组，差异有显著性（F=7．320、6．950，q=4．318-5．897，P〈0．05）。缓解组化疗前RP高于化疗后（t=2．806，P〈0．05）；未缓解组化疗前RP低于化疗后（t=3．128，P〈0．05）。7例初治AL病人骨髓抑制期RP、RP％低于骨髓恢复期（F=7．061、9．345，q=3．932～5．983，P〈0．05）。AL病人RP％与PLT呈负相关（r=-0．373，P〈0．05）。结论 RP可反映AL病人的不同病理状态，也是化疗后骨髓恢复的一个早期且敏感指标。

CHANGE OF RETICULATED PLATELETS AND ITS CLINICAL SIGNIFICANCE IN ACUTE LEUKEMIA

Objective To explore the change of reticulated platelet(RP) in acute leukemia(AL) and to evaluate its significance. Methods RP and PLT were detected and observed during chemotherapy in 32 patients with different type of AL,which were compared with 32 normal controls. Results The RP and PLT in initially-treated and refractory or relapse AL patients were lower than normal controls(F=7.320,6.950;q=4.318-5.897;P<0.05).Before chemotherapy,the RP of AL,before remission,was higher than after chemotherapy(t=2.806,P<0.05),while the RP without remission,before chemotherapy,was lower than after chemotherapy(t=3.128,P<0.05).The RP and RP% of seven initially-treated AL,in myelosuppression stage,were both lower than in recovery phase(F=7.061,9.345;q=3.932-5.983;P<0.05).The RP% of AL was negatively related to PLT(r=-0.373,P<0.05). Conclusion RP can reflex the different status of AL,and is an earlier and sensitive index to predict the recovery of bone marrow after chemotherapy.
