螺旋CT扫描对小细胞肺癌诊断及分期的临床应用价值

目的探讨螺旋CT扫描在小细胞肺癌的诊断价值及分期中的应用。方法搜集67例经病理证实的小细胞肺癌胸部螺旋CT扫描，观察其胸部螺旋CT表现及分期特点。结果67例小细胞肺癌中，中心型54例（80．6％），周围型13例（19．4％）。属于局限性病变27例，广泛播散性病变40例。54例中心型小细胞肺癌中，伴阻塞性肺炎9例，伴阻塞性肺不张5例。50例肺门淋巴结增大，41例纵隔淋巴结增大。21例纵隔大血管被包埋或侵犯。13例周围型小细胞肺癌中，5例肿大的肺门、纵隔淋巴结较肺内肿瘤大。结论小细胞肺癌的螺旋CT扫描有一定特征，螺旋CT在小细胞肺癌的诊断及分期中有较高的应用价值。

Value of spiral CT in diagnosis of small cell lung cancer

Objective] To evaluate the diagnostic significance and staging of spiral CT in small cell lung cancer(SCLC). Methods] Thoracic helical CT findings of 67 patients with pathologically proved SCLC were analyzed. Results] Of 67 cases , central type was seen in 54 (80. 6 %) and peripheral type in 13 (19. 4 %), the lesion was localized in 27 and extensively disseminated in 40. Of 54 central type cases, obstructive pneumonia was accompanied in 9 and atelectasis in 5 cases. Hilar and mediastinal lymphadenopathy was seen in 50 and 41 cases respectively. Involvement or encasement of great vessels in mediastinum occurred in 21 cases. Hilar and mediastinal lymphadenopathy are bigger than lung tumor in 5 cases of 13 peripheral type cases. Conclusion] SCLC has specific CT findings, and CT scanning is useful in the diagnosis and staging of SCLC.
