体外循环心脏不停跳下心内直视手术的临床评价

摘    要:目的 总结25例体外循环心脏不停跳下心内手术的应用经验,对其优缺点进行临床评价。方法 选择我院1999年25例先天性心脏病不停跳手术组为实验组,全组年龄4~15岁,均为非补片修补患者;随机抽取1998~1999年度相同年龄段的心脏停跳非补片修补的先天性心脏病患者25例,作为对照组。对术中体外循环时间、术后气管插管拔除时间、术后24h液体出入情况及引流量、以及术后住院天数进行比较。结果 实验组本外循

Abstract:Objective To summarize the practical experience of open heart surgery in the beating heart during cardiopulmonary bypass in 25 cases. Methods The experimental group included 25 cases of congenital heart disease operated with the heart beating. 25 cases of patients treated with unpatch closure operated in open heart without beating (the same age range as experimental group) were treated as control group. The following index were compared between the two groups: time of CPB, preserving time of trachea intubator, liquid negative balance within 24 h after operation, drainage volume within 24 h after operation, hospitalization days. Results Significant difference existed only in CPB time between the experimental group and the control group (P=001). Other index showed no significant difference between the two groups. Conclusion Open heart surgery in beating heart can simplify the surgical pattern, shorten operating time, diminish the cardiac damage during operation, reduce the post-surgical complications, such as conduction blockage, post-surgical leakage etc. The effect of surgery in beating heart has no difference as compared with that in heart of no beating. This surgical pattern can direct the development and practice of minimal invasive surgery.
