呼吸内科住院病例病原体感染与疾病相关因素分析

目的]了解呼吸内科病原体感染发生的情况以及与疾病相关情况,为医院合理使用抗生素,减少二重感染发生率提供指导.方法]选择我院呼吸内科2012年1月至2013年1月520例住院患者资料进行分析,主要对病例的血液、尿液以及痰液标本进行细菌培养,并且对标本进行药敏分析.观察细菌感染主要存在的部位,对抗生素的敏感情况以及导致患者病原体感染的影响因素.结果]1.呼吸内科患者的细菌感染主要是分布在痰液,其次是中段尿,最后是血液.2.痰液中细菌感染主要是革兰氏阴性菌,肺炎克雷伯菌,大肠杆菌,假单胞菌以及真菌白色念珠菌等.3.呼吸内科发生以上细菌感染的患者疾病主要为肺炎,COPD,支气管哮喘以及肺癌.4.影响病原体感染导致疾病的主要因素有：年龄超过50岁、使用糖皮质激素、免疫力低下、抗生素使用不合理、伴有心脑血管基础疾病医疗器械交叉感染以及病房环境.结论]呼吸内科住院患者发生细菌感染,与患者自身情况,医院环境以及诊疗过程均有关系,临床医生在治疗过程中应充分考虑以上因素的共同作用,以减少感染的发生.

Analysis to Disease Relative Factors and Pathogen Infection of Hospitalized Cases in Respiratory Medicine

Objective] To understand the respiratory medicine pathogen infection situation and relation to the disease,for hospital and respiratory medicine reasonable nse of antibiotics,reduce double infection chance to provide guidance.Methods] Choose 520 cases of hospitalized patients to analyze our hospital respiratory medicine in January 2012-January 2013.We tested main cases for blood,urine and sputum specimens for bacterial culture and antibiotic sensitivity,analysis was carried out on the specimen.Observe the existence of the main parts of the observed bacterial infections,pathogen infection in patients with sensitive antibiotics and cause of influencing factors.Results] （1） Respiratory medicine in patients with bacterial infection mainly occurred in the sputum; Followed by midway through the urine,the last was the blood.（2） In the sputum bacterial infection,mainly gram-negative bacteria,klebsiella pneumoniae,escherichia coli,pseudomonas and fingns candida albicans.（3） The respiratory medicine occurred more in patients with bacterial infection disease with main behavior of pneumonia,COPD,bronchial asthma and lung cancer.（4） The main factors influencing the pathogen infection led to disease for persons over age 50,using glucocorticoid,belonged to the people of low immunity,with basic medical cross infection disease of heart head bloodvessel and ward environment.Conclusion] In patients with respiratory medicine hospital bacterial infections,mainly with the patient＇s own situation,hospital environment and the diagnosis and treatment process,clinical doctors should fully consider in the process of treatment the combination of the above factors,which can reduce the incidence of bacterial infections.
